Effectiveness of Autogenous Platelet Rich Fibrin as Internal Matrix
Introduction: Lateral ridge augmentation has become a standard treatment option to enhance the bone volume of deficient recipient sites prior to implant placement. For successful lateral ridge augmentation, appropriate bone grafting filler materials and barrier membranes are essential. Platelet-Rich Fibrin (PRF) belongs to a new generation of platelet concentrates, with simplified processing. PRF accelerates tissue cicatrization due to the development of effective neovascularization, accelerated wound closing with fast cicatricial tissue remodelling, and nearly total absence of infectious events. The aim of this study was to determine the effect of PRF membrane application on lateral ridge augmentation in the canine mandible.
Materials and Methods : The study was conducted in eight dogs. P1 to P4 premolars were removed and six lateral ridge defects (length 10 mm, height 8 mm, depth 8 mm) were made for each dog. After 4 weeks, re-entry surgery has been performed. On the experimental site augmentation of lateral ridge defects were changed to chronic bone defects in the mandibular alveolar ridge. Experimental sites received tricalcium phosphate (CERASORB) granules or Xenograft (Bio-oss) particles. Three Barrier membranes (resorbable membrane Bio-guide, non-resorbable membrane Cytoplast membrane TXT-200, and PRF) were placed for graft protection. These approaches were compared to ridge augmentation using different bone materials and membranes. After a healing period of 8 weeks, the sites were analyzed histologically and with multi image analysis.
Results : PRF showed better new bone formation and bone density than Bio-guide and Cytoplast membrane TXT-200. Bio-oss sites with PRF membrane protection showed excellent healing results with a well-preserved ridge profile, whereas CERASORB grafts with Cytoplast TXT-200 membrane clearly limiting the treatment outcome.
Conclusion : Autogenous Platelet Rich Fibrin is a effective internal matrix material as a barrier membrane when it comes to using on chronic lateral ridge augmentation.
