Purpose: To determine the relationship between diaphragmatic humps on a chest radiograph and dome hepatocellular carcinoma (HCC), and to identify the clinical significance of diaphragmatic humps in patients with HCC.
Materials and methods: The institutional review board approved this retrospective study and waived the requirement for informed consent. 429 patients who underwent transcatheter arterial chemoembolization (TACE) for HCC treatment were included. We analyzed chest radiographs to detect diaphragmatic humps on the right hemidiaphragm and angiographic images obtained during TACE to detect the corresponding dome HCC. The prevalence of diaphragmatic humps and its corresponding dome HCC was calculated. And the diagnostic performance of detection dome HCC using diaphragmatic humps on chest radiographs was assessed.
Results: The prevalence of diaphragmatic humps was 34.3 % and the frequency of corresponding HCCs was 32.0 %. Sensitivity, specificity, positive and negative predictive values were 85.5 %, 73.3 %, 32.0 % and 97.2 %, respectively. The positive and negative likelihood ratios were 3.2 and 0.2, respectively
Conclusion: A considerable number of dome HCC appeared as diaphragmatic humps and the diaphragmatic hump could be a finding of dome HCC in patients with HCC.
