아토피피부염은 영유아에게서 발병률이 높은 알레르기 질환 중 하나로, 유전, 환경, 식품 등 다양한 요인에 의해 발생하며 식품과의 연관성이 높다. 또한 까다로운 식행동은 유아기에 흔한 섭식문제이며, 아토피피부염의 유발요인이 될 수 있으므로 이들의 예방과 치료에는 영양관리가 필요하다.
본 연구는 2012년 9월부터 2012년 11월까지 삼성서울병원 알레르기 센터에서 아토피피부염으로 진단받은 환아 100명(만 2세 이상 5세 이하)을 대상으로 하였다. 대상자의 일반사항, 영유아의 기질 및 식행동, 어머니의 식행동 및 식사지도 방법은 설문을 통해서 조사하였고, 식이섭취조사는 식품섭취 빈도 조사지(Food Frequency Questionnaire, FFQ)를 이용하여 질병관리본부의 한국인 유전체 역학조사 영양도구(ver. 1.0)로 영양섭취를 분석하였다.
영유아의 식행동 점수를 군집 분석하여 cluster 1을 까다롭게 먹지 않는 영유아(Non-picky eater; NPE)로, cluster 2를 까다롭게 먹는 영유아(Picky eater; PE)로 명명하였다. 연구대상자는 NPE군 53명, PE군 47명이었다. 성별은 NPE군에서 남아가 더 많았고(NPE군 77.4%, PE군 53.2%) PE군에서 여아가 더 많아(NPE군 22.6%, PE군 46.8%), 두 군 간의 유의적인 차이를 보였다(p<.05). 연령은 만 2세가 NPE군(52.8%)과 PE군(38.3%) 모두에서 가장 많았다. 연구대상자의 식욕은 두 군간의 유의적인 차이를 보였는데(p<.001), 식욕이 ‘아주 좋다’의 경우 NPE군 45.3%, PE군 8.5%로 NPE군에서 압도적으로 많았으며, 식욕이 ‘나쁘다’의 경우 PE군 9.1%, NPE군 3.8%로 PE군에서 더 많았다. 수면 시간은 NPE군이 평균 10.8±1.2시간이고 PE군이 평균 10.3±1.0시간으로, NPE군의 수면 시간이 PE군의 수면 시간에 비해 유의적으로 길었다(p<.05). 식품 섭취 능력은 PE군이 NPE군에 비해 '단단한 식품을 잘 씹어 먹는다'의 점수가 유의적으로 낮았다(p<.05). PE군에서 '단단한 식품을 잘 씹어 먹는다'는 '음식을 잘 깨물어 먹는다'(r=.477, p<.01), '음식을 잘 삼킨다'(r=.306, p<.05)와 유의적인 양의 상관관계를 보였고, '손으로 음식을 잘 집어 먹는다'는 '음식을 입에 넣고 입술을 다물고 씹는다'(r=.424, p<.01)와 유의적인 양의 상관관계를 보였다. 식사도구 사용 능력에서 컵, 숟가락, 빨대의 사용 능력은 각각에서 서로 모두 양의 상관관계를 보였다.
영유아의 기질 점수는 '활동수준'이 PE군에서 유의적으로 높게 나타났고(p<.05), '적응성'은 PE군에서 유의적으로 낮게 나타났다(p<.01). 까다롭게 먹는 영유아가 될 확률은, 영유아의 활동이 많고(OR=2.97, CI=1.46-6.02) 낯선 환경에 잘 적응하지 못할 때(OR=0.40, CI=0.19-0.86) PE군이 될 확률이 유의적으로 높았다. 영유아의 기질과 까다롭게 먹는 영유아의 식행동 간의 관계를 분석한 결과, 기질적 규칙성(p<.01)과 기질적 접근성(p<.05)은 식행동의 규칙성과 유의한 양의 상관관계를 보였고, 기질적 접근성은 식행동의 예민함(p<.05)과 유의한 양의 상관관계를 보였다. 어머니의 식행동 점수는 NPE군과 PE군 간의 유의적인 차이가 없었으나, 어머니의 식행동과 까다롭게 먹는 영유아의 식행동간의 관계를 분석한 결과, 어머니의 규제적 식행동은 영유아의 접근적 식행동과(p<.05) 유의한 음의 상관관계를 보였다. 어머니의 식사지도 방법의 점수는 두 군 간의 유의적인 차이가 없었으나, 어머니의 식사지도 방법과 까다롭게 먹는 영유아의 식행동 간의 관계를 분석한 결과, 어머니의 규제적인 식사지도 방법은 영유아의 규칙적 식행동(p<.01)과 유의한 양의 상관관계를 보였고, 강제적 식사지도 방법은 영유아의 규칙적 식행동(p<.05), 식사 시 주의집중도(p<.01)와 유의한 음의 상관관계를 보였다. 모니터링 식사지도 방법은 영유아의 식사 주의집중도(p<.05)와 유의한 양의 상관관계를 보였다.
연구대상자의 한국인 영양섭취기준에 따른 영양소 섭취 비율(%KDRIs)은 두 군 간 유의적인 차이가 없었으며, 칼슘(NPE, 103.3%; PE, 102.5%)과 엽산(NPE, 103.4%; PE, 108.7%)에서 두 군 모두 권장섭취량에 근접하게 섭취하는 것으로 조사되었다. 그러나 에너지 섭취는 NPE군과 PE군이 각각 154.9%, 153.5%로 두 군 모두 필요추정량 대비 과다한 열량을 섭취하고 있는 것으로 나타났으며, 단백질(377.1%, 351.2%), 인(198.4%, 200.2%), 철(150.5%, 146.4%), 아연(246.3%, 237.2%), 비타민 A(137.9%, 128.5%), 비타민 B1(201.5%, 201.3%), 비타민 B2(166.9%, 164.8%), 비타민 B6(247.7%, 239.3%), 나이아신(215.3%, 201.6%), 비타민 C(256.0%, 304.3%), 비타민 E(136.9%, 140.6%)의 섭취율은 권장섭취량(비타민 E의 경우 충분섭취량) 대비 모두 125% 이상으로 과다하게 섭취하고 있는 것으로 나타났다. %KDRIs을 75% 미만, 75% 이상으로 나누어 비교해 본 결과, NPE군과 PE군에서 각 영양소 섭취 비율(%KDRIs)의 유의적인 차이는 나타나지 않았다. 반면 %KDRIs을 125% 미만, 125% 이상으로 나누어 비교해 본 결과, 나이아신은 NPE군(90.6%)이 PE군(74.5%)보다 권장섭취량 대비 125% 이상으로 섭취하는 비율이 유의적으로 높은 것으로 나타났다(p<.05). %KDRIs의 평균을 연령별로 나누어 비교해 본 결과, 연령이 만 5세인 경우 PE군에서 단백질, 칼슘, 인, 아연, 비타민 A, 비타민 B6, 나이아신의 섭취 비율이 NPE군에 비해 유의적으로 낮았다(p<.05). 그러나 그 중 단백질, 인, 아연, 비타민 B6, 나이아신의 섭취는 NPE군과 PE군에서 모두 권장량 대비 125% 이상을 섭취하는 것으로 나타났고, 칼슘 섭취는 PE군에서 권장량 대비 66.4%로 부족하게 섭취하고 있는 것으로 나타났으며, 비타민 A는 108.6% 섭취하므로 권장섭취량에 근접하게 섭취하고 있는 것으로 나타났다. 연령이 만 3세의 경우, 비타민 C의 섭취 비율은 PE군에서 522.5%, NPE군에서 197.3%로 PE군에서 유의적으로 많이 섭취 하는 것으로 나타났다(p<.001). 또한 엽산의 섭취 비율은 PE군에서 153.8%, NPE군에서 88.0%로 PE군이 유의적으로 많이 섭취하였으며(p<.01), 비타민 E의 섭취 비율의 경우도 PE군에서 177.3%, NPE군에서 114.0%로 PE군의 섭취가 유의적으로 높았다(p<.05).
본 연구는 아토피피부염 환아에서 까다로운 식행동을 보이는 영유아의 기질 및 어머니의 식행동과 식사지도 방법의 특성에 대해 조사하였으며, 까다로운 식행동을 보이는 아토피피부염 환아의 식생활지침 마련과 영양교육 및 상담을 위한 기초자료를 제공하고자 한다. 또한 까다로운 식행동을 보이는 아토피피부염 환아를 위한 식품 섭취에 대해 정확한 기준정보의 제공과 체계적이고 지속적인 영양관리가 필요할 것으로 사료된다.

Atopic dermatitis is one of allergic disease that occur the high prevalence of atopic dermatitis during infants and young children. Atopic dermatitis is occurred by various factors such as genetic predisposition, exposure to environmental allergens, or exposure to food antigens. Atopic dermatitis has high correlation with foods. In addition, picky eating is a common eating problem in infants and can be a factor causing atopic dermatitis. For the reason, many infants with atopic dermatitis and picky eating require nutrition care for prevention and treatment.
This study was conducted with 100 infants (ages of two to five) who had been diagnosed with atopic dermatitis in the Samsung Medical Center from September to November 2012. A survey was implemented to investigate general characteristics of subjects, infants’ temperaments and eating behaviors, mothers’ eating behaviors and child-feeding practices; as for dietary intake, the FFQ (Food Frequency Questionnaire) was used to analyze nutrient intake with nutrition research tools for Korean dielectric mechanism of KCDCP (Korea Center for Disease Control and Prevention).
Scores of infants' eating behaviors were analyzed by cluster analysis. Cluster 1 was named non-picky eater (NPE) group and cluster 2 was named picky eater (PE) group. fifty-three infants of the subjects were NPE group, forty-seven infants were PE group. Male infants for NPE group accounted for 77.4%, which was higher than those of PE group at 53.2%, while female infants accounted for 46.8% of PE group, which was higher than those of NPE group at 22.6% with significant difference (p<.05). There were most two-year-old infants both NPE (52.8%) and PE group (38.3%). Having a ‘good appetite’ in NPE group (45.3%) was significantly higher than that in PE group (8.5%) and having a ‘bad appetite’ in PE group (9.1%) was significantly higher than that in NPE group (3.8%) (p<.001). Bed time was average 10.8±1.2 hours per day in NPE group and 10.3±1.0 hours per day in PE group; bed time in NPE group was significantly longer than that in PE group (p<.05). Score of ‘chew solid food well’ of oral motor skill in PE group was significantly lower than that in NPE group (p<.05). In PE group, ‘chew solid food well’ showed significant positive correlation with ‘bite food well’ (p<.01) and ‘swallow food well’ (p<.05) of oral motor skill. In addition, ‘pick up food with fingers’ showed significant positive correlation with ‘chew food with lips closed’ (p<.01) of oral motor skill. ‘Cup’, ‘spoon’ and ‘straw’ of using utensil skill were positively correlated with each other in PE group.
Score of ‘activity level’ of infants’ temperament in PE group was significantly higher than that in NPE group (p<.05) and ‘adaptability’ of infants’ temperament in PE group was significantly lower than that in NPE group (p<.01). ‘Rhythmicity’ (p<.01) and ‘approach-withdrawal’ (p<.05) of infants’ temperament were positively correlated with ’Rhythmicity’ of infants’ eating behavior, ‘approach-withdrawal’ of infants’ temperament was positively correlated with ’pickiness’ (p<.05) of infants’ eating behavior in PE group. As for scores of mothers’ eating behavior, there was no significant differences between NPE and PE groups; ‘restraint’ of mothers’ eating behavior was negatively correlated with ’approach-withdrawal’ (p<.05) of infants’ eating behavior in PE group. As for scores of mothers’ child-feeding practice, there was no significant differences between NPE and PE groups; ‘restriction’ of mothers’ child-feeding practice was positively correlated with ’rhythmicity’ (p<.01) of infants’ eating behavior, ‘pressure’ of mothers’ child-feeding practice was negatively correlated with ’rhythmicity’ (p<.05) and ‘attention span’ (p<.01) of infants’ eating behavior in PE group. ‘Monitoring’ of mothers’ child-feeding practice was positively correlated with ’attention span’ (p<.05) of infants’ eating behavior in PE group.
As for the average nutrient intake as compared with %KDRIs for the subjects, there was no significant differences between two groups. Calcium (NPE, 103.3%; PE, 102.5%) and folic acid (NPE, 103.4%; PE, 108.7%) were having nutrient intake close to recommended nutrient Intake, but estimated energy requirement (154.9% and 153.5% respectively), protein (377.1%, 351.2%), phosphorus (198.4%, 200.2%), iron (150.5%, 146.4%), zinc (246.3%, 237.2%), vitamin A (137.9%, 128.5%), vitamin B1 (201.5%, 201.3%), vitamin B2 (166.9%, 164.8%), vitamin B6 (247.7%, 239.3%), niacin (215.3%, 201.6%), vitamin C (256.0%, 304.3%) and vitamin E (136.9%, 140.6%) were exceeding to recommended nutrient Intake (RNI) (in case of vitamin E; adequate Intake (AI)). As for nutrient intake 125% below/above %KDRIs, there was significantly more infants with intake of niacin 125% above RNI in NPE group (90.6%) than in PE group(74.5%) (p<.05). As for the average nutrient intake as compared with %KDRIs for the subjects by age, PE group had lower intake of protein, calcium, phosphorus, zinc, vitamin A, vitamin B6 and niacin compared to NPE group with significant difference (p<.05) in five-year-old infants; however protein, phosphorus, zinc, vitamin B6 and niacin intake were exceeding 125% to RNI between two groups, calcium(66.4%) was less intake in contrast to RNI and vitamin A(108.6%) was similar to RNI in PE group. In case of three-year-old infants, PE group had higher intake of vitamin C (NPE, 197.3%; PE, 522.5%) (p<.001), folic acid (NPE, 88.0%; PE, 153.8%) (p<.01) and vitamin E (NPE, 114.0%; PE, 177.3%) (p<.05) compared to NPE group with significant difference.
This study investigated about real state of the characteristics of picky eaters’ temperaments, mothers’ eating behaviors and child-feeding practices in infants with atopic dermatitis, and intends to provide basic data for nutrient education, consultation and preparing guidelines for dietary life of picky eaters in infants with atopic dermatitis. In addition, it is considered that the need for providing of correct standard information about taking food for picky eaters in infants with atopic dermatitis and nutrition care systematically and continuously.
