목적: 다발골수종 치료제인 bortezomib과 연관된 마이봄샘기능부전 1예에 대하여 보고하고자 한다.
증례요약: 59세 여자 환자가 두 달 전부터 심해진 안구건조증과 세안을 해도 닦이지 않는 눈꺼풀 분비물을 주소로 내원하였다. 환자는5개월 전 다발골수종을 진단받고 bortezomib 치료를 네 차례 받은 상태였다. 세극등현미경검사에서 양안 눈꺼풀에 다수의 마이봄샘개구부 막힘이 관찰되었고, 마이봄샘촬영 영상에서 위눈꺼풀은 1/3 이상 2/3 미만, 아래눈꺼풀은 2/3 이상의 마이봄샘이 폐쇄되어있었다. 온찜질을 포함한 눈꺼풀 위생, 점안항생제와 스테로이드, 인공눈물 사용 후에도 8개월간 증상과 병변은 큰 차이를 보이지않았다. Bortezomib 치료 종료 2개월 후 환자가 안과에 재내원하였을 때, 주관적 증상은 호전되었고, 마이봄샘 개구부 막힘은 모두사라졌다. 양안의 마이봄샘폐쇄 정도는 큰 차이를 보이지 않았다.
결론: 임상의는 bortezomib 치료를 받는 환자에서 마이봄샘기능부전이 발생하거나 악화될 수 있음에 대해 인지하여야 하고, 마이봄샘기능부전의 치료 방침을 수립하거나 환자를 교육할 때 이를 고려해야 할 것으로 생각한다.
