종합병원 간호사의 전자의무기록 시스템 만족도가 간호업무성과에 미치는 영향 분석 연구 : 서울특별시 일개 대학병원 간호사를 대상으로

국문 초록 (Abstract)

본 연구는 서울시 소재의 일개 대학병원에서 간호사가 전자의무기록시스템을 사용하는 중 그 성능과 기능에 대한 만족도가 임상에서 간호업무성과에 어떻게 영향을 미치는지를 분석하였다.
연구 대상은 전자의무기록시스템을 사용하는 임상간호사 310명이었다. 자료 분석은 설문을 통해 수집된 자료를 SPSS 18.0 통계 패키지를 이용하여 빈도분석, 신뢰도분석, 요인분석 을 실시하였으며, AMOS 20.0 통계 패키지를 이용하여 요인간의 관계를 분석하는 구조방정식 모형분석을 진행하였다.
대상자의 일반적인 특성은 31세 이상 35세 미만이 120명(38.7%), 현재 근무지 경력에서 3년 이상 5년 미만이 132명(42.6%), 총 임상경력에서 3년 이상 5년 미만이 80명(25.8%), 총 EMR사용기간은 5년 이상이 101명(32.6%), 종이기록사용여부에서 없음이 144명(46.5%), 근무부서에서는 병동이 203명(65.5%), 학력은 대졸이상이 178명(57.4%), 직위는 일반간호사가 252명(81.3%) 으로 가장 많았다.
리커르트 5점 척도기준으로 측정한 측정변수의 기술적 특성은 EMR 성능이 3.45점, EMR 기능이 3.78점으로 성능과 기능에 대한 만족도가 보통 이상으로 측정되어 전자의무기록시스템은 어느 정도 임상간호사들에게 정착되어 있는 것으로 판단되었다. 간호성과에 대해서는 독자적간호성과가 3.63점, 비독자적간호성과가 3.90점으로 독자적간호성과보다는 비독자적 간호성과를 더 높게 측정한 것으로 나타났다.
요인별 신뢰도 및 타당도 분석 결과 cronbach's ɑ 값은 0.7이상이며, KMO-Bartlett's Test 값은 0.5이상이고, 누적분산설명률이 0.6기준으로 이상이 없음을 확인하였다. 본 연구모형의 전반적인 적합도에서는 CMIN/DF(2.815), AGFI(0.842), RMR(0.027), RMSEA(0.077), CFI(0.921), PNFI(0.733) 같은 지수들을 확인한 결과 대체로 적합도의 판단기준을 충족시켰다고 판단된다.
연구모형을 분석한 결과 전자의무기록시스템 성능에 대한 만족이 전자의무기록시스템 기능의 만족도에 유의한 영향(표준화계수 0.769)을 미치는 것으로 나타났고, 전자의무기록시스템 성능 및 기능에 대한 만족도가 독자적간호업무성과에 각각 유의한 영향(0.498, 0.290)을 미치는 것으로 나타났다. 반면에 성능 및 기능에 대한 만족도가 비독자적간호업무성과에 각각의 영향(0.029, 0.157)은 있으나 유의하지 않은 것으로 나타났다. 마지막으로 독자적간호업무성과는 비독자적간호업무성과에 유의한 영향(.668)을 미치는 것으로 나타났다.
이러한 결과에 의해 간호사의 전자의무기록시스템 사용이 간호업무성과에 도움이 되었다는 것을 확인한 이상, 병원의 경영진에서는 전자의무기록시스템 사용에 대한 효과를 높이기 위하여 지속적인 개발이 필요할 것으로 생각된다. 기관 자체 내 전자의무기록시스템의 특성 및 효율적인 활용방안, 중요성 및 정보제공에 대하여 정기적인 프로그램 교육도 진행하여야 할 것이다. 간호사들도 간호업무수행에 도움이 되고 병원 간 경쟁의 우위에 설수 있다는 인식을 가지고 시스템 및 정보 활용에 대하여 개인적인 시간과 노력을 들여야 할 것이다.

다국어 초록 (Multilingual Abstract)

The study analyzed how the satisfaction of nurses about performance and function of electronic medical record system on their nursing performance in clinical scenes at general a hospital in Seoul.
  The Subject of study was total of 310 clinical nurses who are using electronic medical record system. The collected data through survey was analyzed into frequency analysis, reliability analysis, and factor analysis through SPSS 18.0 statistics package and was analyzed into structural equation model through AMOS 20.0 statistics package.
  General characteristics of the subjects were 120 age between 31 and 35 (38.7%), 132 work experience in the current work site from 3 years to 5 years (42.6%), 80 with total clinical work experience between 3 to 5 years(25.8%), 101 with total of EMR usage periods over 5 years(32.6%), 144 with no experience of paper medical record system (46.5%), 203 nurses who work at ward (65.5%), college graduated (57.4%) as the most common education level, and 252 general nurses (81.3%) as the most common work position.
  Descriptive characteristics of measurement variables measured by Likert 5 type scale showed that clinical nurses are settled with electronic medical record system since the satisfaction rate about performance and function were measured higher than the average as EMR performance score was 3.45 and EMR function score was 3.78. Dependent nursing outcome was shown to be higher than that of independent nursing as the independent nursing outcome score was 3.63 and dependent nursing outcome score was 3.90.
As the result of analysis of reliability rate and validity rate by factors, it was shown that Chronbach's ɑ value was higher than 0.7 and KMO-Bartlett's Test results were higher than 0.5 and cumulative variance explaned proportion had no abnormality on 0.6 point basis. In overall goodness of fit of the study model, it was considered that the study model satisfied standard of judgment after checking variables such as CMIN/DF(2.815), AGFI(0.842), RMR(0.027), RMSEA(0.077), CFI(0.921), PNFI(0.733).
As the result of the analysis of study model, it was shown that satisfaction about the performance of electronic medical record system had significant effect on the function of the electronic medical system (beta=0.769), each satisfactory level about performance and function ha significant effect on independent nursing performance (0.498, 0.290) However, it was shown that each satisfactory level about function and performance did not have significant effect on the dependent nursing performance even though there were some effect (0.029, 0.157). At last, independent nursing performance outcome was shown to have a significant effect on dependent nursing performance outcome (0.668).
Since it was proved that the use of electronic medical record system was shown to be helpful for nursing performance outcome through the result of the study, hospital administrators should develop continuously in order to increase effect of the use of the electronic medical system in hospitals. Plans for effective use of electronic medical record system in facilities and periodical educations about the importance of the system and about the use of the program should be performed at the same time. Also, the nurses should put private time and efforts to the use of the system with an aware about nursing and about the fact that the hospitals can be put into superior condition during the competitions among hospitals.
