      Background: We investigated whether low serum adiponectin concentrations are able to predict the future development of type 2 diabetes and metabolic syndrome. Methods: This was a prospective cohort study, which included 372 elderly Koreans that participated in the South-West Seoul(SWS) study, conducted in 1999 and 2002 in Seoul, Korea. Fasting and post-challenge 2-hour plasma glucose, body mass index(BMI), waist-to-hip ratio(WHR), blood pressure, lipid profiles, and serum adiponectin data were examined. Results: Adiponectin concentrations obtained in 1999 and 2002 were highly correlated(r=0.63, P<0.0001), and the three-year within-person variation of the adiponectin concentrations was not significant(P=0.61). The serum adiponectin level was closely correlated with metabolic syndrome; negatively with BMI, WHR, blood pressure, triglyceride and blood glucose, and positively with HDL cholesterol. Subjects with metabolic syndrome showed lower serum adiponectin concentrations than those without(P<0.0001). The baseline adiponectin levels were found to be correlated with subsequent changes in the WHR, LDL cholesterol, and fasting and post-load 2h glucose over the 3-year period, after adjusting the baseline values. A multiple logistic regression analysis showed that lower baseline serum adiponectin concentrations were significantly associated with the developments of type 2 diabetes and the metabolic syndrome after adjusting for age, gender, obesity, history of impaired fasting glucose or impaired glucose tolerance, hypertension and dyslipidemia.
      Conclusion: Reduced concentrations of adiponectin were found to be independently associated with increase risks of both type 2 diabetes and metabolic syndrome in elderly Koreans

      연구배경: 근래의 연구 결과 비만한 사람에게 혈청 아디포넥틴의 농도가 감소되어 있는 것이 발견되었고 제2형 당뇨병이나 인슐린저항성, 심혈관계 질환 등에서도 아디포넥틴 농도의 감소...

      연구배경: 근래의 연구 결과 비만한 사람에게 혈청 아디포넥틴의 농도가 감소되어 있는 것이 발견되었고 제2형 당뇨병이나 인슐린저항성, 심혈관계 질환 등에서도 아디포넥틴 농도의 감소가 관찰되었다. 본 연구에서는 한국 노인 인구에서 혈청 아디포넥틴의 농도가 제2형 당뇨병을 비롯하여 대사증후군의 발병을 예측할 수 있는 지 알아보고자 하였다.방법: 1999년에 SWS(South-west Seoul) 연구에 참가한 건강한 60세 이상의 노인 대상자 중 2002년에 추적 검사가 완료된 372명을 대상으로 3년간의 공복 및 당부하 2시간 혈당, 체질량지수, 허리-엉덩이둘레비, 혈압, 혈중 지질 및 아디포넥틴 농도를 측정하여 비교하였다.결과: 3년 동안 혈청 아디포넥틴 농도의 개체내변이는 유의하지 않았으며(P=0.61) 혈청 아디포넥틴의 농도는 심혈관계 질환의 위험 인자와 밀접한 연관성이 있어 체질량지수, 허리-엉덩이둘레비, 혈압, 중성 지방, 그리고 혈당과는 음의 상관관계를, 고밀도 콜레스테롤과는 양의 상관관계를 보였다. 대사증후군을 가진 대상 인구는 낮은 혈청 아디포넥틴 농도를 보였으며 (P<0.0001), 다중 로지스틱 회귀분석 결과 낮은 혈청 아디포넥틴 농도는 제2형 당뇨병과 대사증후군의 발병과도 의미 있는 연관성을 보였다.
