하악골 골절후 발생한 외상성 경동맥-해면 정맥동루 = Traumatic Carotid-Cavernous Fistula as a sequela of Mandibular Fracture

국문 초록 (Abstract)

경동맥-해면 정맥동루는 두 개 안면 손상후 발생하는 합병증 중에서 발생하는 비율이 드물지만 뇌신경 손상, 시력 상실, 심지어 두개강내 출혈 등이 발생할 수 있다. 경동맥-해면정맥동루의 치료는 구강악안면 외과의 영역을 넘어서는 것이지만, 경동맥-해면 정맥동루가 발생한 경우 즉각적인 진단과 치료가 이루어질 수 있도록 하여야 한다. 따라서 구강악안면외과의는 악안면 손상 환자에 있어서 손상의 경중과 관계없이 이러한 합병증의 가능성을 항상 염두에 두어야 한다.

다국어 초록 (Multilingual Abstract)

Traumatic catotid-cavernous fistula(CCF) is an uncommon situation resulting from trauma. The blood shunted from the internal carotid artery to the cavernous sinus results in pulsation exophthalmos, orbital headache, bruit, loss of visual acuity, diplopia, ophthalmoplegia.
We were recently faced with such a case. The patient sustained fracture of the mandible during sports activity. Five months later, all the classic signs and symptoms of CCF had appeared. A carotid angiography confirmed the presence of a CCF. The management was achieved intravascularly by platinum electothrombosis embolization. The main signs and symptoms of the CCF had resolved, but the patient remained in a visual loss of affected eye.
