Purpose: The purpose of study was to identify how patients experienced chemotherapy-induced peripheral neuropathy (CIPN) and quality of life related to CIPN. Methods: This was a descriptive research. We collected data from 105 patients with chemotherapy-induced peripheral neuropathy. They completed a self-reported questionnaire including EORTC (Eastern Cooperative Oncology Group) CIPN20 and items related to their disease and peripheral neuropathy. The investigators filled in part of items about disease and treatment. Results: In the study, duration of peripheral neuropathy was 9.4month and 54.3% of patients used pharmacological or non-pharmacological interventions. Patients reported the highest score for sensory scale and it’s score was 38.74±20.24. The scores for motor scale and autonomic scale were 21.95±19.19and 26.61±21.0 respectively. This showed that patients more suffered from sensory neuropathy than any other domain of neuropathy. The most frequently selected two items were ‘did you have tingling fingers or hands?’ and ‘did you have tingling toes or feet?’. Conclusion: The results of this study will provide useful information for chemotherapy-induced peripheral neuropathy.

본 연구는 항암화학요법을 받는 암환자가 경험하는 말초신경병증의 특성을 살펴보고 말초신경병증과 관련된 삶의 질을파악하고자 시도된 조사연구이다.
연구기간은 2009년 10월 15일부터 2010년 3월 2일까지로서울시내 S병원 혈액종양내과 외래 방문 또는 입원한 환자로항암제치료를 받았거나 받고 있으면서 이후에 말초신경병증이발생한 것으로 파악된 환자를 대상으로 하였고 모두 105명의환자가 참여하였다.
조사는 말초신경병증 관련 삶의 질(CIPN20)도구가 포함된설문을 작성하는 방법으로 진행하였다. 수집된 자료는 SPSS (window용 version 12.0)를 이용하여 실수와 백분율, 평균,표준편차로 산출하였고 t-test, ANOVA와 Pearson’s Correlation Coefficient를 이용하여 검증하였다.
본 연구의 결과는 다음과 같다.
첫째, 대상자의 말초신경병증 평균 지속기간은 평균 9.4개월이었으며 항암화학요법 횟수는 7.5회였다. 말초신경병증 조절을 위한 중재방법으로는 약물중재를 받고 있는 대상자가 14.3%,대상자 스스로 비약물 중재를 사용하고 있는 경우가 31.4%이었으며, 약물과 비약물 중재를 동시에 사용하고 있는 경우가 8.6%였다.
둘째, 말초신경병증 관련 삶의 질(CIPN 20)의 점수는 감각 영역이 38.7점으로 가장 높았고, 운동 영역이 21.9점으로 가장 낮았으며 자율신경 영역은 26.6점이었다. 문항별로 증상 비율을살펴보면 감각 영역에서는‘손이나 손가락에 저린 느낌’의 비율이 가장 높았고 운동 영역에서는‘다리의 허약감’, 자율신경 영역 중에는‘발기가 되거나 유지하는데 어려움’이 가장 높았다.
셋째, 말초신경병증 관련 특성들과 말초신경병증 관련 삶의질은 유의한 상관관계를 보이지 않았다.
본 연구는 다양한 암질환과 치료이력을 가진 환자를 대상으로 한 조사연구로는 대상자의 수가 적은 제한점이 있다. 그러나본 연구에서 나타난 결과들은 임상현장에서 항암화학요법으로인해 발생하는 말초신경병증의 특성을 이해하는데 도움을 줄것으로 기대된다. 또한 본 연구결과를 바탕으로 향후 좀 더 많은 환자를 대상으로 한 반복연구와 말초신경병증의 효과적인중재방법 등에 대한 체계적인 연구가 진행된다면 암환자가 경험하는 말초신경병증의 특성을 좀더 명확하게 파악하여 중재하고 결과적으로 암환자의 삶의 질을 향상시키는데 도움이 될것으로 기대된다.
