혈변을 동반한 위장관 유전분증 1예 = A Case of Gastrointestinal Amyloidosis Presenting with Hematochezia

Amyloidosis is a disorder of protein metabolism that leads to extracellular deposition of amyloid in tissues and organs. Multiple organ dysfunction can be induced by deposition of amyloid. Diagnosis is established by histologic demonstration of amyloid protein in involved tissue using Congo red staining. A 72 year-old woman was admitted due to mild abdominal pain, diarrhea and hematochezia. Duodenoscopic and colonoscopic findings were nonspecific, however, multiple biopsies showed amyloid deposits characterized by green birefringence under polarized light. The findings of small bowel series and abdominal CT were compatible with amyloidosis. We experienced a case of amyloidosis with involvement of the whole bowel. 국문 초록 (Abstract)

유전분증은 조직의 세포외 공간에 유전분 침착으로 인해 생기는 다양한 질환을 통칭하는 용어이다. 유전분의 침착으로 인해 여러 장기의 기능장애가 나타나며, 따라서 다양한 임상적 증상들이 나타나게 된다. 저자들은 경한 복통과 설사와 혈변을 주소로 내원한 72세 여자 환자에서 내시경적 조직검사로 유전분증을 진단하였다. 십이지장 내시경, 대장 내시경검사에서 모두 유전분증에 부합하는 육안 및 병리 소견을 보였으며, 소장 조영술과 복부 전산화 단층촬영 결과 역시 유전분증에 부합하는 소견을 보였다. 소화기계 이외의 다른 장기의 침범은 확인할 수 없었다. 저자들은 혈변을 주소로 내원한 환자에서, 위장관만을 침범한 유전분증을 경험하였기에 보고하는 바이다.
