Malignant transformation of Warthin's tumor is extremely rare, although it is the second most common benign tumor of the parotid gland. Adenocarcinoma has infiltrative, perineural and intravascular growth features that indicate a malignant potential. The peak incidence of age is in the fifth decade of life, and there is no gender predilection. We describe our experience of adenocarcinoma arising in Warthin's tumor diagnosed by the excision of a parotid mass in 64-year-old man. The tumor was removed by total parotidectomy with preservation of the facial nerve. The postoperative pathology report revealed that only one lymph node was metastasized and no capsular invasion was found. The patient has no facial palsy and recurrence of disease for 1 year since the operation.
