노인 대상의 의료기기 임상 시험 및 부작용에 대한 피험자의 인지도 조사 = How Aware Elderly Subjects are of Medical Device Clinical Trials and Their Adverse Events - A Survery

Background: Along with the recent greater number of medical devices for the elderly in the market, there has been a parallel increase in the number of clinical trials for these devices. It is uncertain, though, whether the target population has the awareness and understanding of these clinical trials and the possible adverse events of the devices.
Methods: A total of 147 elderly subjects (mean age, 65.8±8.3 years; 39 males) responded to the questionnaire. A pilot study during the development of the questionnaire was conducted in 2009 with the actual survey taking place from June to November 2011. The questionnaire included 22 items-10 items on awareness and understanding of the clinical trial; 4 items on the understanding of adverse events; and 8 items on demographical characteristics.
Results: Of the responders, 62.5% had participated in at least one clinical trial and had heard about the clinical trials through a doctor, newspaper, or television. And 63.3% of the responders understood that the purpose of the trial was to show efficacy and safety for the medical device. There were significant differences on the awareness of the trial by gender and life level. However, there was no statistically significant difference in adverse events awareness.
Conclusion: Although elderly subjects participate in clinical trials, it seems they do not receive enough information when involved in clinical trials of medical devices. In the future, before these trials are carried out, medical device companies need to ensure that they provide their elderly subjects with better education and information.

국문 초록 (Abstract)

연구 배경: 최근 국내에서 의료기기 산업의 발전 및 고령화사회로 노인 대상 의료기기 임상 시험이 활성화되고 있다.
그러나 국내의 경우 의약품 임상 시험에 대한 인지도는 연구는있으나 의료기기 임상 시험에 대한 인지도 조사 연구는 부족하며 특히 노인들의 의료기기 임상 시험에 대한 인지도와 의료기기 부작용에 대한 인지도 조사 연구는 없는 실정이다. 따라서본 연구에서는 의료기기 임상 시험에 대한 인지도 조사를노인을 대상으로 시행하여 의료기기 임상 시험에 대한 인지현황을 파악하고자 하였다.
방법: 본 연구는 147명의 노인들을 대상으로 의료기기임상 시험과 부작용에 대한 인지도를 조사한 연구이다. 연구를위한 설문지 작성을 위하여 2009년 예비 조사를 시행하였고그 내용을 기반으로 22개 문항으로 구성된 설문지는 여러차례에 걸쳐 의료기기 임상 시험 센터 및 부작용 보고 자문위원단으로 구성된 전문가들로부터 검토를 받았다. 이 설문지를이용하여 의료기기 임상 시험에 대한 인지도 조사를 2010년6월부터 2011년 11월까지 두 차례에 걸쳐 설문 조사를 시행하였다.
결과: 의약품 임상 시험에 대한 인지도에 비해 의료기기임상 시험의 인지도가 통계적으로 유의하게 낮은 것으로 나타났다. 의료기기 임상 시험 경험에 따른 ‘의료기기 임상 시험용어 들은 경로’, ‘의료기기 임상 시험의 목적’에 따라 유의하게차이가 나타났으나 ‘의료기기 임상 시험의 대상’에 대한 질문에서는 유의한 차이가 없었다. 성별에 따른 ‘의료기기 임상시험의 목적’에 대한 차이가 나타났으나 학력 수준에 따른차이는 없었다. 생활 수준에 따라 ‘의료기기 임상 시험의 용어를 들은 경로’에 대한 차이가 나타났고, 종교에 따른 차이는없었다. 의료기기 임상 시험 인지 여부와 의료기기 부작용 인지 여부에는 통계적으로 유의한 차이는 없었다.
결론: 의료기기 임상 시험에 대한 인지도가 낮은 것으로조사되어 지속적인 홍보와 안내가 필요할 것으로 보인다.
특히 의약품의 경우에 비하여 의료기기 임상 시험의 인지도가낮은 것은 임상 시험의 규모가 작으므로 예상된 결과이지만,의료기기 산업이 부가가치가 높고 친고령 의료기기 산업의발전 양상을 고려하면 노인군을 대상으로 한 의료기기 임상시험 인지도 제고가 시급하다.
