고령장애인의 개념 정립과 복지욕구 비교를 통한 돌봄 서비스 정책방향 설정에 대한 고찰

국문 초록 (Abstract)

본 연구는 인구 고령화 흐름과 더불어 급속히 증가하는 그 규모에도 불구하고 현재 노인복지와 장애인복지의 분화적 발달 속에 오히려 사각지대에 놓이게 되는 고령장애인에 대한 돌봄서비스 정책 개발의 방향성을 모색하고자 실행되었다. 65세 이상의 고령장애인은 개념상 ‘노인성 장애노인’과 ‘고령화된 장애인’으로 구분되어지는데, 장애와 고령이라는 이중적 위험에도 불구하고 초기 정책설계에서 오히려 노인장기요양보험제도와 장애인 활동지원제도 적용의 사각지대에 놓이게 되었다. 따라서 본 연구의 전반부에서는 2011년 전국노인실태조사와 장애인실태조사를 비교하여 사회서비스 관련 욕구들을 분석하였다. 연구결과 ‘고령화된 장애인’은 소득보장 영역에서, ‘노인성 장애노인’의 경우는 주로 의료보장 욕구에 대한 우선순위가 강함을 알 수 있었다. 따라서 장애기간이 길수록, 발생시기가 어릴수록 ‘소득보장’에 대한 욕구가 의료보장보다 강하다는 점을 알 수 있었다. 특히 고령장애인 중에서도 ‘고령화된 장애인’에 대한 접근과 ‘노인성 장애노인’에 대한 복지욕구 해결 접근에는 분명한 차이점을 두어야 함을 알 수 있었다. 본 연구 두 번째 부분에서는 장애인활동지원제도와 노인장기요양제도라는 고령장애인의 주요 돌봄 서비스간의 적절한 연계 방안 모색을 위해 일본의 사례 및 좋은 돌봄 관련 문헌을 분석하여 고령장애인에 대한 돌봄 서비스 정책의 함의점을 논의하였다. 두 제도의 연계 및 정책적 지원방향 논의에서 증가하게 될 고령장애인에 대한 욕구에 부합하는 서비스 유형 개발 및 대상자 자격 등의 조정 등을 통해 고령장애인의 복지 욕구에 충족되는 서비스 지원이 절실히 필요하다고 하겠다.

다국어 초록 (Multilingual Abstract)

The purpose of this paper is to examine the special welfare needs of the elders with disability and to suggest the integrated policy of aligning the relationship between National Long-term Care Insurance(NLTCI) for Elders and Personal Assistance Services(PAS) for the disabled. In spite of the increasing needs of ‘aging’ and ‘disability’ of the elders with disability, both NLTCI and PAS policies have not considered special needs of elders with disability. The key contents of this study were as follows. In the first section, this study discussed and analyzed welfare-related characteristics of elders with disability compared with those of the entire elderly population by using the data of 2011 National Survey of Elders and 2011 National Survey of the Disabled established by The Korean Institute of Health and Social Affairs (KIHASA). In the second section, with focusing the elders with disability, the study discussed the integrative relationships NLTCI for Elders and PAS for the disabled. The study analyzed the similar cases of Japan and USA policies for the long-term care and welfare services for the disabled. Based on these two sections of the discussions, the study concluded that both fields of the elderly welfare and the disability welfare tried to find integrative ways of covering special needs of ‘aging’ and ‘disability’ for elders with disability.
