가구소득과 아동기의 건강 불평등 = Family Income and Child Health Gradient in Korea

국문 초록 (Abstract)

소득과 건강 간의 관계는 중요한 보건정책관련 이슈이며 그간 경제학 및 기타 사회과학분야에서 이루어진 연구들을 통해 성인건강상태와 소득 간에는 정의 상관관계가 있음이 밝혀졌다. 그러나 소득이 건강으로 연결되는 구체적인 전달과정에 대한 연구는 역인 과관계의 문제로 인해 합의된 결과에 도달하지 못한 상태이다. 본 연구는 역인과관계문제의 해결을 위한 한 방편으로 성인기에 나타나는 건강에서의 소득 불평등을 발생시키는 주요 요인의 하나로서 가구소득과 아동건강의 관계에 대해 분석한다. 이를 위해 한국 복지패널의 만 0세-17세 아동들을 대상으로 주관적 건강상태로 측정한 건강상태와 가구 소득 간의 관계를 임의효과 패널순위프로빗모형을 이용하여 추정한다. 추정결과 아동건강은 가구소득과 양의 관계를 가지며 더 나아가 이러한 관계의 강도가 아동연령의 증가와 함께 증가하다가 청소년기에 약간 감소하는 것으로 나타난다. 이렇게 양의 관계가 나타나는 이유를 분석한 결과 아동이 만성질환을 보유하는 경우 이에 대한 대처가 가구 소득별로 다르기 때문인 것으로 드러난다. 즉, 만성질환을 보유하는 저소득층의 아동은 만성질환을 보유하는 고소득층의 아동에 비해 건강이 좋지 않은 것으로 나타난다. 결과적으로 저소득층 아동의 경우 고소득층 아동에 비해 좋지 않은 건강상태와 학력상태를 가지고 성인기를 맞이할 가능성이 높으며 이를 통해 성인기의 소득과 건강 간의 관계의 단초가 마련된다고 판단한다.

다국어 초록 (Multilingual Abstract)

The relationship between income and health is an important policy related issue and a large empirical literature in economics and other social sciences has provided evidence that there is a positive association between them in adulthood. However, the mechanisms by which income is related to health still remain controversial due to the reverse causality problem. This article investigates the relationship between family income and child health to find out the origin of the positive association between health and income in adulthood. For that purpose, we estimate a random effect panel ordered probit model for children whose age ranges from 0 to 17 using Korea Welfare Panel Study. In Korea, not only children`s health is positively related to household income, but also the relationship becomes more pronounced with children`s age up to 13 years old stage and slightly decreases afterwards. Part of the positive relationship can be explained by the impact of chronic conditions. Children from lower income households with chronic conditions tend to have relatively worse health than those from higher income households do. As a result, poorer children enter the adulthood period with worse health status and less education and this may result in the positive association between income and health in adulthood.
