뺨과 아래팔에 발생한 망상 홍반성 점액증 1예 = A Case of Reticular Erythematous Mucinosis on Cheeks and Forearms

국문 초록 (Abstract)

망상 홍반성 점액증(reticular erythematous mucinosis, REM)은 드문 만성 피부병으로 만성적인 망상 홍반과 관련된 피부 점액증이며 주로 가슴상부와 등의 중심부에 위치하고 드물게는 얼굴, 팔, 복부, 서혜부에서 발견되기도 한다. REM의 발병기전과 병인은 현재까지 밝혀지지 않았으며, 병리조직 소견 상 진피내에 림프구의 침윤과 점액의 침착이 있다. 이에 저자들은 5개월 전부터 39세 여자의 양쪽 뺨과 아래괄에 발생한 지속적이고，경한 소양감의 홍반성 피부 발진이 병리조직학적으로 REM에 일치하고 임상사진에 근거하여 REM 1예로 진단하고 항말라리아제로 치료하였으며, 병변의 거의 완전한 소실과 부작용이 없는 드문 예로 생각되어 문헌 고찰과 함께 보고한다.

Reticular erythematous mucinosis (REM) is a rare chronic dermatosis, in which cutaneous musinosis is associated with chronic reticular erythema, mostly located on the central part of upper chest and back. Less commonly, lesions can bo found on the face, arms, abdomen, and groins. The pathogenesis and etiology of REM remain undefined at present. A mild to pronounced lymphocytic infiltrate and mucin deposition in dermis are characteristic histologic feature. We herein report on a case of REM on cheeks and forearms, successfully treated with antimalarials with emphasis on the clinical and histopathologic findings.
