Acne is more concerned because it can result in redness, pigmentation and scars. Moreover, scars may be permanent that are surprisingly difficult to treat. Various methods have been applied to treat scars as follows: laser resurfacing, chemical peeling, microdermabrasion, maual dermasanding, skin rolling, punch elevation, punch graft, invasive treatment such as subcision, fillers, noninvasive lasers such as smooth beam, and ablative or nonablative fractional lasers. Picosecond laser is well known for the treatment of tattoos. However, recently, holographic fractional handpiece has been developed to broaden the prospective of scar treatment. Primary photo-acoustic effect of picoseconds pulses create microscopic damage to areas within tissue below epidermal level sparing the epidermis and cause a wound-healing response that leads to neocollagenesis and elastin. Thus, we believe that combining of picoseconds laser and subdermal minimal surgery technique which is a novel therapeutic modality may deliver effective and efficient results with the treatment of acne scars.
