Danggwisu-san (DGSS, 當歸鬚散, Dangguixu-san) is an herbal formula that has been mainly used in the Far East Asia for the treatment of pain induced by bruise, sprain and external injury. The cause of this pain is that qi and blood become tangled and do not circulate well. DGSS can improve the tangled situation and make it well-circulated. DGSS was originated from Uihagimmun (醫學入門; Yixuerumen), composed of 9 herbs such as fine root of Angelicae Gigantis Radix, Paeoniae Radix, Linderae Radix, and so on. According to previous studies DGSS has the effects of tissue damage recovery, intestinal motility improvement, blood vessel relaxation, pneumonia improvement, etc. But, there is few study regarding on the effects of DGSS in the immunological activities and its mechanism. The present study evaluated the anti-inflammatory effects of DGSS on Raw 264.7 cells and in rats paw edema model.
Cell viability was measured using the MTT assay. The amount of nitric oxide (NO) production was measured the amount of nitrite content in the cultured medium using Griess reagent. The amount of tumor necrosis factor-α (TNF-α), monocyte chemoattractant protein 1 (MCP-1), interleukin (IL)-1βand IL-6 in the cultured supernatant were measured by ELISA kit.
In addition, the effects of DGSS on the proteins expression about nuclear factorκB (NF-κB) and mitogen-activated protein kinases (MAPK) pathways were detected by Western blot. Furthermore, the effect of DGSS on acute inflammation was observed in rat paw edema model.
The DGSS ameliorates the lipopolysaccharide (LPS)-activated changes in NO production, iNOS expression and pro-inflammatory cytokines and chemokine contents. Additionally, DGSS significantly suppressed expression of p-JNK, p-ERK and nuclear NF-κB. As expected, in rat paw edema study, 1.0 g/㎏ of DGSS significantly reduced the carrageenan (CA)-induced paw edema and iNOS expression for 1~4 h. Moreover, administration of 1.0 g/㎏ (4 days) of DGSS used in this study did not show any significant change on ALT and AST.
These results demonstrate that DGSS reduced inflammatory mediators in LPS-activated Raw 264.7 cells through the suppression of the NF-κB and MAPK pathways (JNK and ERK, not p-38). In addition, the results of the in vivo CA-induced paw edema model indicate that DGSS could ameliorate an inflammatory
mediator and edema. Therefore, this present study can put scientific evidences up for the anti-inflammatory effect of DGSS as well as the underlying mechanisms.

當歸鬚散은 <醫學入門> 外集卷之七에 記載된 方劑로 打撲而致氣凝血結하여 胸腹脅痛或寒熱을 치료한다. 또한 <醫學入門> 七卷 通用古方時括 折傷에서도 當歸鬚散은 折傷을 치료한다고 하였고, <한의임상진료지침>에서도 급성 족관절 염좌에는 ‘當歸鬚散’, ‘淸熱瀉濕湯’등의 한약을 권고하고 있다.
當歸鬚散에 대한 연구로는 瘀血과 손상회복, 皮下血腫에 미치는 연구, 打撲負傷된 흰쥐의 혈액 효소활성도에 미치는 영향 등이 보고되었다. 외상과 염증의 관계는 外傷으로 염증이 발생하기도 하고, 또한 外傷後 感染에 의해 염증이 발생하기도 한다. 본 연구에서는 當歸鬚散이 外傷後 瘀血뿐만아니라 外傷後 수반될 수 있는 炎症 반응을 억제할 수 있는지, ipopolysaccharaide (LPS)에 의해 활성화된 macrophage cell line인 Raw 264.7 cell에 當歸鬚散 추출물을 처치하여 염증매개 물질의 발현 억제효능을 평가하고, 또한 carrageenan (CA)로 유도한 족부종 rat에 當歸鬚散 추출물을 경구 투여하여 부종의 정도 및 염증매개물질의 발현억제를 평가하였다.
시험관 연구에서, 세포 생존율은 MTT 분석법을 사용하였으며, 산화질소(NO)의 생성량은 Griess 시약을 이용하여 배양배지 내 아질산염 함량을 측정하였다. 종양괴사인자-α(TNF-α), 단핵구화학유인단백질 1(MCP-1), 인터루킨(IL)-1β 및 IL-6의 양은 ELISA 키트로 측정하였다. 또한, DGSS가 핵인자-κB(NF-κB) 및 미토겐활성화단백질키나제(MAPK) 경로에 대한 단백질 발현에 미치는 영향을 Western blot을 통해 평가하였다. 동물모델로는 CA로 유도된 급성 염증모델에 대한 DGSS의 효과를 평가하였다.
DGSS는 LPS에 활성화된 NO 생성, iNOS 발현, 전염증성 사이토카인 및 케모카인 함량을 유의하게 억제하였으며, 또한, DGSS는 p-JNK, p-ERK 및 p-I-κB의 발현을 유의하게 억제하였다. Rat의 족부종 연구에서는 1.0 g/㎏의 DGSS는 CA로 유발된 족부종과 iNOS 발현을 1~4시간에 유의하게 감소시켰다. 더불어 본 동물실험기간 (4일) 및 투여용량 (최대 1.0 g/㎏)에서 DGSS는 ALT, AST에 유의한 변화를 나타내지 않았다. 이러한 결과는 DGSS가 NF-κB 및 MAPK 경로의 억제를 통해 염증반응을 개선할 수 있음을 나타낸다.
본 연구의 결과가 當歸鬚散 구성약물 중 어느 하나의 단독적인 효능인지, 아니면 특별 조합의 효능인지는 평가하기가 불가능하지만, 當歸鬚散은 주치증인 攧撲墜落 壓倒傷 등의 諸傷에서 氣凝血結로 인한 痛症외에 직간접적으로 유도될 수 있는 염증에 대하여 억제작용을 가지므로, 임상에서 活血祛瘀 理氣止痛의 목적에 더하여 抗炎症에 대한 목적을 부가할 수 있는 객관적인 근거를 제공할 것으로 생각된다.
