A deforming, non-erosive, non-destructive and painless arthritis has been called Jaccoud`s arthropathy, which is similar to the deformity of rheumatoid arthritis. Hand deformities associated with parkinsonism were reported before the era of levodopa treatment. It suggests that Jaccoud`s arthropathy associated with parkinsonism may result from dystonia and muscular rigidity. We encountered a 69 year-old female patient with idiopathic parkinsonism and Jaccoud`s arthropathy in her hands. In the left fourth and fifth digits, the proximal interphalangeal(PIP) joints were hyperextended and the distal interphalangeal(DIP) joints were flexed(swan neck deformity). She also had ulnar deviation and flexion deformity of the metacarpophalangeal(MCP) joints in the left hand. The hand deformities showed passive reducibility. The joints did not reveal any evidence of synovitis on examination and radiographs. Rheumatoid factor was negative. We described a case of Jaccoud`s arthropathy in a parkinsonian patient, and review the literatures.
