Early records says that a sick bikku took a pūtimuttabhesajja (陳棄药) which is generally known as a ‘medicine’ made by fermenting bovine urine and shit. Some literary resources add that the drug is the product of fermenting bovine urine and poop, human urine and shit, or bovine urine alone. Chinese translators and the later Buddhist scholars were aware that pūtimuttabhesajja is a ‘medicine’ using bovine or human urine, however, no further details of its substance has been known or found so far.
Only fragments of explanation on the drug can be found in some scritptures, so it is difficult to draw a clear picture of it. The notion of a drug made from the animal refuse is highly unlikely to be accepted by outsiders or later generations who do not understand the culture of the time.
Such an example can be found in the Namhaegkiguinebopzon (南海寄歸內法傳) of Yijing (義淨) in 7th century Tang Dynasty, At that time in China, people put human faeces and urine in a bowl, called it ‘Yong-tang (龍湯)’, and took it as a medicine. He criticized harshly, and wanted to correct the custom by giving a proper example of the pūtimuttabhesajja used in India.
The origin of pūtimuttabhesajja can be found in India’s traditional folk remedies of which the urine therapy takes a big part. This paper tried to find the meaning and substance of the pūtimuttabhesajja through investigating the case of using cow urine and its efficacy in traditional Indian medicine.

진기약이란 불교에서 출가비구가 병에 걸렸을 때 먹는 약이다. 불교경전을 비롯해서 비구의 생활 규범을 담고 있는 율장에서도 진기약에 대한 설명은 매우 부실하다. 출가비구들이 아플 때에는 소의 오줌과 똥으로 만든 진기약을 사용했다고 하는 것이 일반적인 견해이다. 그러나 율장에서는 병에 걸린 비구가 진기약 외에도 다양한 약용 음식을 사용하고 있으며, 진기약의 복용여부에 의문이 생길만큼 병에 따른 다양한 치료법에 대해서 기술하고 있다.
율장에서 진기약이 언급되는 곳은 약과 관련된 곳이 아니라 출가비구의 입단의식에서 다룬다. 진기약은 이상적인 출가사문의 생활상으로 제시하고 있는 사의지 가운데 하나인 것이다. 달리 말하자면 부연 설명을 하지 않아도 이해할 수 있는 일상적인 상비약이라는 의미가 된다.
인도불교의 전통과 문화를 벗어나자 그 실체를 정확하게 파악하지 못했던 중국의 역경가나 후대의 학자들은 진기약에 대한 설명을 덧붙일 수밖에 없었고, 오히려 오해가 생겨났다. 7세기 당나라의 구법승인 의정(義淨)의 저서인 『남해기귀내법전(南海寄歸內法傳)』에서당시 중국불교가 진기약의 실체를 모르고 사람과 동물의 똥과 오줌으로 용탕을 제조하여 약으로 쓰는 관행을 언급하고 있다. 의정은 진기약의 원어인 pūti-mukta- bhaiṣajya와 pūtimuttabhesajja의 분석을 통해 진기약을 소의 오줌으로 만든 약[腐尿藥]이라고 정의한다.
진기약의 재료로 언급되는 소의 오줌은 인도의 전통의학에 있어 가장 널리 사용된 약재였다. 소의 오줌을 이용한 요료법은 피부병과 같은 간단한 질병부터 암에 이르기까지 매우 광범위하게 사용된 치료법이었다. 엄격한 수행생활을 했던 수행자 혹은 부처님 당시 출가수행을 했던 초기의 불교수행자들이 고가의 재료와 까다로운 법제를 통해 약을 구한 다는 것은 있을 수 없는 일이다. 따라서 인도 전통의학에서 사용된 소의 오줌이 바로 진기약이라고 보아도 무방할 것이다.
