정책옹호연합모형(ACF)을 통한 정책변동 분석: 건강보험 보장성 강화정책을 중심으로 = Analysis of Policy Changes based on the Advocacy Coalition Framework: Focusing on Health Insurance Coverage Expansion Policies

국문 초록 (Abstract)

건강보험 보장성을 강화하기 위한 정책이 역대 정부에서 지속해서 추진되었음에도 우리나라의 건강보험 보장률은 여전히 목표치에 미달한 60% 중반대에 정체되어 있다. 본 연구는 정책옹호연합모형(Advocacy Coalition Framework)을 통해 이러한 건강보험 보장성 정책의 변동과정을 분석하고, 과정에서 중요하게 작용한 변동 요인이 무엇이었는지, 그리고 그로 인한 정책의 결과까지 체계적으로 분석하고자 한다. 구체적으로 노무현 정부부터 박근혜 정부시기까지 건강보험 보장성 강화정책의 변화 과정을 살펴보고, 정책변동을 둘러싼 외생변수, 장기적 연합 기회구조, 정책하위체제의 요소를 확인하고 설명했으며, 그 결과 나타난 정책변동의 내용을 제시하였다. 분석결과, 사회구조의 변화와 여론의 변화가 건강보험 보장성 강화정책을 추동하였음을 알 수 있었으며, 반면 정책중개자의 활동과 정책지향학습으로 인한 정책변동은 두드러지지 않았다. 아울러 본 연구는 옹호연합의 구성과 신념체계, 전략과 자원도 분석하였다. 결론에서는 연구의 의의와 한계, 후속 연구의 방향을 제시하였다.

다국어 초록 (Multilingual Abstract)

Although policies to improve health insurance coverage have been continuously promoted by previous governments, the health insurance coverage rate in Korea is still stagnant in the mid-60% range, which is below the target value. The purpose of this study is to analyze the process of changes in health insurance coverage expansion policies from the Roh Moo-hyun administration to the Park Geun-hye administration through the Advocacy Coalition Framework (ACF), and systematically analyze the factors of change that played an important role in the process, and the policy results. Based on the ACF, exogenous variables surrounding policy change, long-term coalition opportunity structures, and elements of the policy sub-system were identified and explained, and the details of the policy change that occurred as a result were presented. As a result of the analysis, it was confirmed that changes in social structure and public opinion drove policies to expand health insurance coverage, while the policy changes due to activities of policy brokers and policy-oriented learning were not prominent. Moreover, this study analyzed the composition of the advocacy coalition, belief systems, strategies, and resources. In conclusion, the significance, limitations, and direction for follow-up studies were presented.
