거동이 불편한 환자를 대상으로 한 홈헬스케어써비스의 인식도와 요구도에 관한 연구 = Cognition and Needs for Home Health Care Services on the Patients with Limited Activities of Daily Life

Objective : To evaluate the baseline need and actual situation for activation of home health care services.
Methods and Materials : The level of cognition and need for home nursing care service and the activities of daily life(ADL) were evaluated in December, 2000 by questionnaire. The subjects were 201 inpatients who have cerebral(36.3％), spinal(26.9％) and musculoskeletal(15.4％) problems at the ward of neurology, neurosurgery, orthopedics and rehabilitation in the five general hospitals at the city of Busan. The data were analyzed with PC SAS(ver 6.12).
Result : About 64.7％ of the patients had no idea, but 90.9％ of them feel need and 95.5％, 97.5％ of them want to get knowledges required for nursing patients at home. Check blood pressure, pulse, body temperature and respiration(77.1％), patient education(60.2％), injection(57.7％), exercise training(55.7％), counselling(43.3％), back and alcohol massage(37.3％) were frequently needed by the patients and their family. And, the need was the highest in the category of education and counselling among three categories - baseline nursing, treatable nursing, and education and counselling. In ADL, they feel much difficulties in use for rest room, bath?shower, walking, going up stairs, moving from bed to wheel chair, etc. On the other hand, they feel less difficulties in washing hands, combing, meals, hand motion, etc. The mean level of ADL with four Likert scale lower in walking(1.56), wheel chair handling(1.75) and personal hygiene(1.86). The mean level of ADL according to the diagnosis group, most of them - bed life, personal hygiene, walking, clothing, wheel chair handling - was the lowest in spinal cases, and that of meal and hands motion was the lowest in cerebral cases.
Conclusion : The authors could image the priority of needs of home health care system, and it considered that spinal cases need more assist than cerebral cases. For activating home health care system, it might be important that raise the level of cognition and encourage the need based services.
