유방암진단에서 유방촬영술, 초음파유방촬영술, $^{99m}$Tc-MIBI스캔을 이용한 다양식 검사(multimodality study)의 유용성 = Diagnostic Usefulness of the Multimodality Study with Mammography, Ultrasonography, $^{99m}$Tc-MIBI Scau in Breast Cancer

국문 초록 (Abstract)

목적: 유방암의 진단에 있어 유방촬영 술, 초음파유방촬영 술, $^{99m}$ Tc-MIBI 스캔을 이용한 다양식 검사(multimodality study)의 유용성 대해서 알아보고자 하였다. 대상과 방법: 1998년 1월부터 1999년 12월까지 유방종괴로 수술을 받은 81명의 환자를 대상으로 하였다. 모든 환자는 유방촬영 술, 초음파유방촬영 술 $^{99m}$ Tc-MIBI 스캔을 시행하였고 환자의 필름을 후향적으로 조사하였다. 각각의 검사에 대한 평가는 조직학적 진단을 모르는 각 2인의 진단방사선과 및 핵의학과 전문의에 의해 grading하였으며 . $^{99m}$ Tc-MIBI 스캔과 초음파유방촬영술은 4단계 grading(1 = definitely benign. 2=probably benign. 3=probably malignnant, 4= definitely malignant)과. 유방촬영술은 BIRADS 를 이용하였다. 유방종괴는 67 예는 수술로, 14 예는 세침생검술로 확진되었고. $^{99m}$ Tc-MIBI 스캔과 초음파유방촬영술에서 grade 3, 4, 유방촬영 술에서 BIRADS category 4, 5인 경우는 검사상 양성 . 다양식 검사에는 두 개, 3개의 검사 모두에서 악성으로 판정되었을 경우만 양성으로 정의하여 각각의 검사와 조합시의 검사의 민감도. 특이도, 양성 예측도. 음성예측도. 정확도를 산출하였다. 결과: 유방촬영 술의 민감도, 특이도. 양성예측도. 음성예측도, 정확도는 각각 87.3%, 69.2% 87.3% , 81.8% . 80.2%이었고, 초음파유방촬영술은 94.5% 65.3% 85.2%. 85.0%, 85.2% 이었고,$^{99m}$ Tc-MIBI 스캔은 83.6% ,88.5% .93.9% . 71.9% .85.2%이었다. 초음파유방촬영술과 유방촬영술 조합의 경우는 83.6%. 50.0%. 93.9%, 100.0%, 95.2%이었고. $^{99m}$ Tc-MIBI스캔과 유방촬영술의 경우는 72.7%, 69.2%. 95.2%, 100.0%, 967%이었고. $^{99m}$ Tc-MIBI 스캔과 초음파유방촬영술의 조합의 경우는 80.0%, 61.5% , 957% , 94.1% , 95.2% 이었고. 세검사를 모두 조합한 경우는 70.9%, 50.0%. 97.5%. 100.0% . 98.7% 이었다. 결론: 유방암의 진단에 있어서 이양식 검사가 검사 하나를 시행했을 경우보다 양성예측도, 음성 예측도, 정확도가 높았고 임상에서 흔히 시행되는 유방촬영 술과 초음파의 조합이 다양식 검사 중 민감도가 가장 높아서 검사의 편이성과 비용 등을 감안하면 유방촬영 술과 초음파의 조합이 임상에서 가장 유용하였다

다국어 초록 (Multilingual Abstract)

Purpose: To assess the diagnostic usefulness of multimodality study in the diagnosis of breast cancer. Materials and Methods: Eighty-one patients between January 1998 to December 1799 were involved in this study, and who underwent mammography, breast ultrasonography, and $^{99m}$Tc-MIBI scintimammography in all cases the findings were retrospectively reviewed. Each modality was graded by two physicians specializing in nuclear medicine and two radiologists, all unaware of the pathologic results. A four-grade system(l = definitely benign, 2=probably benign, 3 =probably malignant, 4=definitely malignant) was applied to those of breast ultrasonography and $^{99m}$Tc-MIBI scintimammography and BIRADS was applied to those of mammography. All breast masses were confirmed by surgery (n= 67) or FNA (n= 14). Findings of grade 3 or 4 the fourgrade system, BIRADS category 4 or 5, or positive coincidence in double and triple combination studies were defined as positive results, and on the basis of the data thus obtained, the sensitivity, specificity, positive and negative predictive value and diagnostic accuracy were calculated for each modality and for multimodality studies. Results: The sensitivity, specificity, positive predictive value, negative predictive value and diagnostic accuracy of $^{99m}$Tc-MIBI scintimammography were 83.6% 88.5%, 93.7%, 71.9% and 85.2%, respectively. For ultrasonography, the corresponding figure were 94.5% ,65.3%, 85.2%, 85.05, and 85.2%, and these for mammography, they were 87.3%, 69.2%, 87.3%, 81.8% and 80.2%. For the ultrasonography and mammography combination, the figures were 83.6%, 50.0%, 73.7%, 100.0% and 95.2%, respectively, and for $^{99m}$Tc-MIBI scintimammography and mammography, the corresponding findings were 72.7%, 67.2%, 75.2%, 100.0% and 76.7%. For the $^{99m}$Tc-MIBI scintimammography and ultrasonography combination, respective findings of were 80.0%,61.5% ,75.7%,94.1% and 75.2%, respectively, and in the triple modality study, respective findings of 70.9%, 50.0%, 97.5%, 100.0% and 98.1% were recorded. recorded.
