난시검사법에 따른 완전교정의 효용성 평가 = Evaluation of the Effectiveness of Full-Correction according tothe Refraction Tests for Astigmatism

Purpose: This study aimed to analyze the effect of differences in prescription according to the test method for astigmatism during refraction on corrected vision. Methods: Twenty-five subjects in their 10s and 30s with no history of vision correction surgery and/or eye disease were tested for astigmatism using a clock dial chart and cross-cylinder, respectively, for their prescription. After wearing the test lens to which each prescription was applied, the minimum angle of resolution (MAR) of the subjects was measured using the Landolt C-ring chart while changing the contrast and letter size, and the difference according to the prescription was further compared. Results: The cross-cylinder test tended to have a higher prescription value for spherical correction, while the test using a clock dial chart tended to have a higher prescription value for cylindrical correction. Comparing the MAR according to the change in the contrast and the letter size, it was found that the MAR was statistically and significantly better when corrected using the prescription of the cross-cylinder test than that of the clock dial chart. In addition, regardless of the contrast, it was found that the smaller the size of the letters, the more statistically significant difference in the number of letters read between the two refraction tests. Furthermore, there was a difference in the number of letters read between the two refraction tests, depending on the contrast of letters; however, the difference did not show a consistent correlation. Conclusions: From the result of this study, it was found that in astigmatism, which mainly involves precision work or far-distance viewing for a long time, precise correction by cross-cylinder test should be performed to improve vision satisfaction.

국문 초록 (Abstract)

목적: 본 연구는 굴절검사 시 난시검사 방법에 따른 처방값의 차이가 교정시력에 어떠한 영향을 미치는 가를 알 아보고자 하였다. 방법: 시력교정술이나 안질환의 이력이 없는 10~30대 25명을 대상으로 각각 방사선 시표와 크로 스실린더로 난시 검사를 시행한 후 처방값을 구하였다. 각 처방값이 적용된 시험렌즈 착용 후 란돌트 C 링 시표를 사 용하여 대비 및 크기를 변화시키며 대상자들의 최소분리시력을 측정하고 처방값에 따른 차이를 비교하였다. 결과: 크 로스실린더 검사 시에는 구면 교정도수 처방값이 강하게 나오는 경향이 있었던 반면, 방사선 시표 검사 시에는 원주 교정도수 처방값이 강하게 나오는 경향이 있었다. 시표의 대비 및 크기 변화에 따른 최소분리시력을 비교한 결과, 크로스실린더 검사의 처방값으로 교정 시에는 방사선 시표 검사의 처방값의 경우보다 통계적으로 유의하게 우수한 최소분리시력을 보이는 것으로 나타났다. 또한, 시표의 대비에 관계없이 시표의 크기가 작아질수록 두 검사방법 사 이의 읽은 시표의 개수 차이가 통계적으로 유의하게 커짐을 알 수 있었다. 또한 시표 대비에 따른 두 검사방법 간 읽은 시표의 개수 차이는 존재하였으나 일정한 상관성을 보이는 차이는 아니었다. 결론: 본 연구 결과 정밀한 작업 하거나 원거리를 장시간동안 주시하는 시생활을 주로 하는 난시안의 경우라면 크로스실린더 검사를 통한 난시의 정 밀교정이 시생활 만족도의 향상에 도움이 될 것임을 알 수 있었다.
