    본 연구는 건강검진 수검자 216명을 대상으로 연령, 성별, 체질량지수(BMI), 고혈압, 당뇨, 고지혈증 등 대사증후군 요인이 갑상선 결절의 존재, 개수, 및 악성도(K-TIRADS)에 미치는 영향을 분석하였다. 갑상선 초음파로 결절 특성을 평가하고, 혈압·혈당·지질을 기준으로 대사증후군을 정의하였다. 분석결과, 연령 증가에 다라 결절 크기(F=5.698, p<0.001)와 개수(F=7.863, p<0.001)가 유의하게 증가하였으며, 여성과 고령이 결절 개수의 주요 예측인자로 확인되었다. BMI는 결절의 존재(χ²=8.803, p=0.012)와 다발성과 연관되었으나 악성도와의 직접적 관련은 없었다. 다변량 로지스틱 회귀분석에서 결절의 악성도는 연령(OR=1.049, p=0.001), 고혈압(OR=8.384, p=0.005), 고지혈증(OR=8.000, p<0.001)과 유의한 양의 연관성을 보였고, 고지혈증은 결절 양상의 독립적 결정요인이었다(OR=3.78, p<0.001). 이는 노화에 따른 갑상선 실질 변성, 순환·지질대사 이상으로 인한 혈관내피 기능 저하, 산화스트레스 및 세포증식 경로 활성화가 결절 병태생리에 관여함을 시사하며 연령, 성별, 고혈압, 고지혈증을 통합 고려한 위험도 평가와 대사 관리가 임상적 결절 예측 및 예방에 유용함이 확인되었다.

    This study analyzed the effects of metabolic syndrome–related factors—including age, sex, body mass index (BMI), hypertension, diabetes, and hyperlipidemia—on the presence, number, size, and malignancy of thyroid nodules among 216 health-screening participants. Thyroid nodule characteristics were evaluated using ultrasonography, and malignancy risk was classified according to the Korean Thyroid Imaging Reporting and Data System (K-TIRADS). Metabolic syndrome components were defined based on blood pressure, fasting glucose, and lipid profiles. Nodule size (F=5.698, p<0.001) and number (F=7.863, p<0.001) significantly increased with age, and both advanced age and female sex were major predictors of nodule multiplicity. BMI was associated with nodule presence (χ²=8.803, p=0.012) and multiplicity but not with higher K-TIRADS categories. In multivariate logistic regression, nodule malignancy risk demonstrated a positive association with age (OR=1.049, p=0.001), hypertension (OR=8.384, p=0.005), and hyperlipidemia (OR=8.000, p<0.001), while hyperlipidemia also independently influenced nodule type (OR=3.78, p<0.001). These findings suggest that thyroid parenchymal degeneration related to aging, along with circulatory and lipid metabolic abnormalities leading to endothelial dysfunction, oxidative stress, and activation of proliferative pathways, may contribute to thyroid nodule pathophysiology. Incorporating age, sex, hypertension, and hyperlipidemia into clinical risk assessment and metabolic management strategies may enhance prediction and prevention of thyroid nodule development.
