Background: Multimorbidity and suicide rates are on the rising among older Korean population. Recent studies have shown that multimorbidity is associated with increased suicidal ideation. Thus, this study was to investigate individual factors of suicidal ideation among older Korean adults with multimorbidity.
Methods: A cross-sectional study was conducted using a subsample (n=20,533) with multimorbidity aged 65 over of the 2017 Korean community health survey. Binary logistic regression model assessed the association of physical activity, sitting times, self-reported sleep duration, and depression with suicidal ideation in older adults with multimorbidity.
Results: Overall, 2,790 (13.6%) of the sample reported suicidal ideation within the past year. Logistic regression analysis showed that women, low monthly family income, low physical activity, long sitting times, inappropriate sleep duration and depression were significantly associated with increased risk of suicidal ideation in older adults with multimorbidity.
Conclusions: To reduce the rate of suicidal ideation in older adults with multimorbidity, healthcare providers should be aware of the importance on routine assessment for physical activities, sleep pattern, and mental status of older adults in clinical settings. Particularly, female and low-income older adults with multimorbidity should be prioritized for early detection and individualized suicide prevention based on our findings.

국문 초록 (Abstract)

연구배경: 인구 고령화와 함께 만성 질환 증가는 노인 연령층에서의 자살률을 가중시키고 있다. 복합 만성 질환 노인의 정신건강 증진과 자살 고위험군 조기 발견을 위해서는자살행동에서 가장 시작단계인 자살생각을 이해하는 것이필요하다. 이에 본 연구에서는 복합 만성 질환을 가지고 있는 노인을 대상으로 자살 생각률과 자살생각 관련요인을 파악하여 복합 만성 질환 노인의 자살계획으로 이어지는 것을전환시킬 수 있는 자살 예방프로그램 개발에 기초자료를 제공하고자 시도되었다.
방법: 연구 대상자는 2017년도 지역사회건강조사에 참여한 전체 대상자 중 고혈압, 당뇨, 관절염 중 2개 이상의 복합만성 질환을 가지고 있는 노인 20,533명이었다. 복합표본 설계에 따른 자료 분석은 IBM SPSS version 26.0을 이용하여기술통계, 교차분석, 상관관계, 로지스틱 회귀분석 모두 복합표본 방법을 이용하여 분석하였다. 결과: 전체 연구 대상자 중 지난 1년간 자살생각을 한 적이 있다고 응답한 대상자는 2,790명(13.6%)이었다. 여성이거나 가구 월소득이 낮은 경우, 신체활동이 적은 경우, 좌식시간이 길어질수록, 수면시간이 부족하거나 길수록 자살을생각할 가능성이 높았다. 특히 우울은 복합 만성 질환을 가진 노인의 자살생각을 높이는 가장 강력한 위험요인으로 나타났다.
결론: 복합 만성 질환을 가진 노인들의 자살생각과 자살을 예방하기 위해서, 우울을 조기에 발견하여 적절한 치료를 받을 수 있도록 돕는 체계화된 보건의료전달 체계 구축과 더불어 신체활동과 수면 질을 향상시킬 수 있는 통합적전략이 함께 마련되어야겠다. 나아가 보건의료전문가들은복합 만성 질환을 가진 노인의 자살위험에 대한 심각성을인지하고, 성별과 경제 수준을 고려한 노인 대상의 자살예방관리시스템 구축 마련에 힘써야겠다.
