비전위성 대퇴경부 골절에 대한 내고정술 후 대퇴골두 무혈성괴사를 예측하기 위해 시행한 골주사 검사 결과를 추시함으로써 수술 후 시간이 지남에 따라 방사성동위원소의 골흡수가 어떻게 변화되는지 파악하고자 하였다.
본원에서 비전위성 대퇴경부골절로 유관나사를 이용한 내고정술 후 골주사 검사를 시행하고 수술 후 3년 이상 추시가 가능했던 54명의 환자를 대상으로 후향적으로 연구하였다. 전례에서 24시간 이내에 수술을 시행하였고 수술 후 2주 이내에 1차 골주사 검사를 시행하였고 술 후 1개월에서 6개월 사이에 2차 골주사 검사와 술 후 12개월에서 18개월 사이에 3차 골주사 검사, 술 후 18개월에서 24개월 사이에 4차 골주사 검사를 시행하였다. 검사 시기별로 대퇴골두비(femoral head ratio) 값을 구하여 시간이 지남에 따라 방사성동위원소의 골흡수가 어떻게 변화하는지 관찰하였다.
수술 후 2주 이내에 1차 골주사 검사를 시행한 54명의 환자에서 대퇴골두비는 평균 0.99 (범위, 0.42-2.04) 였으며, 1차 골주사 검사에서 대퇴골두비 값이 1미만으로 건측에 비해 환측의 방사성동위원소 흡수 감소를 보인 것은 54명 중 38명(70.4%) 이었다. 하지만 방사성동위원소 흡수 감소를 보인 38명의 환자 중 단 1명 만이 대퇴골두 괴사로 진행하였고 나머지 37명의 환자는 1개월에서 6개월 사이에 시행한 2차 골주사 검사에서 골흡수 증가를 보였다. 술 후 1개월에서 6개월 사이에 골주사 검사를 시행한 47명의 환자에서 대퇴골두비 값은 평균 1.69(범위, 0.84-3.80) 이었으며 술 후 12개월에서 18개월 사이에 3차 골주사 검사를 시행한 13명의 환자에서 대퇴골두비 값은 평균 1.29(범위, 1.01-2.62), 술 후 18개월에서 24개월 사이에 4차 골주사 검사를 시행한 8명의 환자에서 대퇴골두비 값은 평균 1.05(범위, 1.03-1.16) 이었다. 또한 시행한 1, 2, 3, 4차 골주사 검사 간의 대퇴골두비 값이 통계적으로 유의한 차이를 보였다(P = 0.035). 3차 이상의 골주사 검사를 시행한 13명을 대상으로 시기별 대퇴골두비 값의 변화를 개인별로 표시한 결과 대체로 수술 직후에는 방사성동위원소 흡수 감소를 보이다가 1-6개월 사이에 흡수 증가가 크게 보이고 술 후 12개월이 경과하면서 점차 흡수증가가 줄어들어 건측의 흡수 정도와 비슷해지는 경향을 보였다.
비전위성 대퇴경부 골절에 대한 내고정술 후 골주사 검사에서 방사성동위원소의 골흡수는 수술직후에 감소를 보이다가 시간이 지남에 따라 일정기간 흡수가 크게 증가되고 다시 서서히 건측과의 흡수 정도와 비슷해지는 독특한 양상을 보인다. 이러한 변화양상을 토대로 생각해 볼 때 수상 직후 및 수술 후 조기 골주사 검사의 결과를 가지고 추후 대퇴골두괴사 여부를 예측하는 것은 부적절하며 연속적인 골흡수 변화양상을 보는 것이 중요할 것으로 생각된다.

To investigate the changing pattern of the uptake of isotope in sequential bone scan test predict osteonecrosis of the femoral head in patients with an undisplaced femoral neck fracture.
54 cases undertaken sequential bone scan for non-displaced femoral neck fracture treated by internal fixation with cannulated screws between 2000 and 2007 were retrospectively studied. The mean follow-up period was 4.2 years (range, 3 to 7 years). All patients underwent gentleclosed reduction and internal fixation with cannulated screws for their femur neck fracture within 24 hours from injury. First postoperative bone scan was taken within 2 weeksafter surgeryin all the patients. Then second, third, and fourth follow-up bone scans were taken at 1 to 6 months, 12 to 18 months, and 18 to 24 months after surgery respectively. We analyzed the changing pattern of the uptake of isotope in bone scan as time passed by comparing femoral head ratios in different postoperative period.
The mean femoral head ratio(FHR) in first postoperative bone scan was 0.99(range, 0.42 to 2.04). Even though the FHR was less than 1.00(cold uptake) in 38 patients(70.4%, 38/54), only one patient had developed osteonecrosis of the femoral head. The others showed hot uptake in their second follow-up bone scan and achieved bone healing without complications. The mean FHR in second, third, and fourth postoperative bone scans was 1.69(range, 0.84 to 3.80), 1.29(range, 1.01-2.62), and 1.05(range, 1.03-1.16) respectively and there was significant differencesstatistically in each follow-up period(P=0.035). In addition, there was similarpattern of uptake of isotope as time passed. The FHR which is less than '1' in early postoperative bone scan gets over '1' at 1 month to 6months after surgery and comesdown around the level '1'at 12 months after surgery in follow-up bone scan.
We should not over-estimate the early postoperative bone scan results to predict osteonecrosis of the femoral head and sequential bone scan is recommended to predict osteonecrosis in undisplaced femur neck fracture because there are unique pattern of uptake of isotope as time passed like that cold uptake in early, hot uptake in a couple of month, and iso-uptake in late.
