임상간호사의 비판적 사고성향, 문제해결과정이 환자안전간호활동에 미치는 영향 = Effects of Clinical Nurses’ Critical Thinking Disposition and Problem-Solving Process on Patient Safety Nursing Activity

국문 초록 (Abstract)

본 연구는 임상간호사의 비판적 사고성향, 문제해결과정이 환자안전간호활동에 미치는 영향을 파악하여 궁극적으로 임상간호사의 의료 서비스 제공과정에서의 환자안전간호활동을 향상시키고 관리하는 기초 자료로 활용하고자 시도된 서술적 조사연구이다.
본 연구의 자료 수집은 2021년 8월 30일부터 9월 15일까지 대전 광역시와 전라북도에 위치한 4개의 100병상 이상의 병원에서 근무하는 임상 경력 6개월 이상의 간호사 203명을 대상으로 자가 보고식 설문지를 이용하여 이루어 졌다. 비판적 사고성향은 27문항으로 구성된 윤진(2004)의 비판적 사고성향 측정도구를 이용하였고, 문제해결과정은 30문항의 이우숙 등(2008)의 성인의 문제해결과정 측정도구를 이용하였다. 환자안전간호활동은 32문항으로 구성된 한미영(2015)등의 환자안전간호활동 측정도구를 이용하였다.
자료의 분석은 SPSS/WIN 25.0 program을 사용하여 기술통계, t-test, ANOVA, Scheffe's test, Pearson's correlation coefficients, stepwise multiple regression 으로 분석하였다.
본 연구의 결과의 요약은 다음과 같다.
1. 임상간호사의 비판적 사고성향 정도는 5점 만점 중 3.63점, 문제 해결과정 정도는 5점만점 중 3.57점, 환자안전간호활동은 5점 만점 중 4.56점 이었다.
2. 임상간호사의 일반적 특성에 따른 환자안전간호활동은 연령 (F=3.203, p=.024), 결혼 상태(t=-2.434, p=.016), 부서(F=2.655, p=.034), 직위(F=9.104, p<.001), 안전교육횟수 (F=3.594, p =.015)에서 유의한 차이가 있는 것으로 나타났다.
3. 임상간호사의 환자안전간호활동은 비판적 사고성향(r=.415, p<.001) 및 문제해결과정(r=.362, p<.001)과 양의 상관 관계를 나타났으며, 비판적 사고성향은 문재해결과정(r=.788, p<.001)과 유의한 양의 상관관계가 있는 것으로 나타났다.
4. 임상간호사의 환자안전간호활동에 영향을 주는 요인은 비판적 사고성향(β=.374, p<.001), 직위(β=.213, p=.001), 인증평가경험 (β=.123, p=.049)의 순이었고, 영향요인의 설명력은 총 22.5%이며, 회귀식은 유의(F=20.537, p <.001)하였다.
본 연구는 임상간호사의 비판적 사고성향과 문제해결과정이 환자안전간호활동과의 관계를 파악하였다는데 의의가 있다. 결과적으로 임상간호사의 환자안전간호활동에 영향을 주는 요인으로는 비판적사고성향과 책임간호사 이상의 직위, 인증평가경험이 긍정적인 요인임을 확인할 수 있었다. 따라서 간호사의 간호활동에 큰 영향을 미치는 간호관리자는 간호사가 환자안전간호활동을 수행하도록 적절한 교육을 제공하고 올바른 역할모델이 될 필요가 있다.

다국어 초록 (Multilingual Abstract)

Introduction : Patient safety nursing activities are systematic management performed by nurses to prevent and reduce risks or accidents. This study is a descriptive study conducted to identify the factors influencing clinical nurses' critical thinking disposition and problem-solving process on patient safety nursing activities.
Methods : The subjects of this study were 203 nurses with more than 6 months of clinical experience in 4 hospitals with 100 beds or more, and critical thinking disposition, problem solving process, and patient safety nursing activities were measured. Data were analyzed by descriptive statistics, t-test, ANOVA, Scheffe's test, Perarson's correlation, and stepwise multiple regression.
Results : In this study, the critical thinking disposition of clinical nurses was 3.63 points, the degree of problem-solving process was 3.57 points, and patient safety nursing activities were 4.56 points. The variables that showed significant differences in patient safety nursing activities of clinical nurses were age (F=3.203, p=.024), marital status (t=-2.434, p=.016), department (F=2.655, p=. 034), job title (F=9.104, p=.001), and number of safety educations (F=3.594, p=.015). Factors influencing the clinical nurse's patient safety nursing activities were critical thinking disposition (β=.374, p<.001), position (β=.213, p<.001), and certification evaluation experience (β=.123, p=.049), the explanatory power of the influencing factors was 22.5% in total, and the regression equation was significant (F=20.537, p<.001).
Conclusion : In this study, it was confirmed that the critical thinking disposition, the position of a senior nurse or higher, and certification evaluation experience were positive factors as factors affecting the patient safety nursing activities of clinical nurses. Therefore, it is necessary to increase the critical thinking disposition in the clinical field to perform patient safety nursing activities. In addition, as it was said that the higher the position, the higher the patient safety nursing activity. Therefore, the nursing manager, who has a great influence on the nurse's nursing activity, needs to provide appropriate education and become a correct role model for the nurse to perform the patient safety nursing activity.
