미숙아 동맥관 개존의 진단에서 혈장 Atrial Natriuretic Peptide 농도의 유용성에 관한 연구 = Usefulness of Plasma Atrial Natriuretic Peptide Concentration in the Diagnosis of Patent Ductus Arteriosus in Preterm Infants

Purpose : Diagnosis of a hemodynamically significant patent ductus arteriosus(PDA) that should be treated is difficult to determine by clinical and echocardiographic examinations. The aim of this study was to clarify the interrelationship of plasma atrial natriuretic peptide(ANP) concentrations and clinical signs or echocardiographic signs of PDA in preterm infants and use plasma ANP concentrations as a diagnostic indicator of a hemodynamically significant PDA.
Methods : Twenty-four preterm infants who were born at Guro Hospital, Korea Medical Center were evaluated on the 3rd day after birth. Clinical signs(cardiovascular dysfunction score ■3) and echocardiographic signs[magnitude of left-to-right shunt across a PDA and left atrium(LA) : Ao ratio] of a PDA and plasma ANP concentrations were measured.
Results : Plasma ANP concentrations and LA/Ao ratios of preterm infants with hemodynamically significant PDA were significantly higher than those of preterm infants without hemodynamically significant PDA. Plasma ANP concentrations were related to the magnitude of the left- to-right shunt across a PDA and LA : Ao ratio. Plasma ANP concentrations of LA/Ao of more than 1.3 were significantly higher than that of LA/Ao of less than 1.3.
Conclusion : In preterm infants, high plasma ANP concentration may be a useful predictive indicator of a hemodynamically significant PDA. Measurement of plasma ANP concentration may play a role in deciding the need for and timing of medical or surgical management of preterm infants with PDA.

국문 초록 (Abstract)

목 적 : 미숙아에서 혈역학적으로 의미있는 동맥관 개존을 진단하여, 약물 요법이나 수술적 치료가 필요한지를 결정하고 그 치료시기를 정확히 판단하는 것이 중요하다. 본 저자들은 혈역학적으로 의미있는 동맥관 개존을 반영하는 객관적인 진단적 지표를 알아보고자 동맥관 개존의 임상적 소견과 심초음파 소견에 따른 혈장 ANP 농도의 차이를 비교하여 그 임상적 유용성을 알아보고자 하였다. 방 법 : 1998년 7월부터 12월까지 고려대학교 의료원 구로병원 신생아실에 입원하였던 미숙아 24명을 대상으로 하였다. 생후 3일째에 심장 초음파 검사로 동맥관 개존을 확인하고 동맥관의 좌우 단락 크기와 좌심방/대동맥 비를 구하였다. Yeh 등의 임상적 진단 기준에 따라 혈역학적으로 의미있는 동맥관 개존의 임상적 소견을 관찰하고 혈장 ANP 농도는 γ-counter로 측정하였다. 심초음파상 동맥관 개존이 없었던 군(8명, N-PDA군), 동맥관 개존은 있으나 임상적 소견이 없었던 군(10명, NS-PDA군)과 동맥관 개존의 임상적 소견이 있어 indomethacin 투여가 필요했던 군(6명, S-PDA군)으로 나누어 각각의 군을 비교 관찰하였다.
