폐암에서 객담의 세포학적 진단에 관한 연구 = The Study on Diagnosis of Cytology in the Lung Cancer

Four hundred and three patients with sputue cytologic examination were presented at Ewha womens University Hospital during the period of 3 years from 1981 to 1983.
1) Age distribution was highest in the age of 60 years, 28.9% and the ratio of male to female in the patients was 1.7 : 1.
2) Among the whole 366 cases differentiated by the Pap. class, the class Ⅰ was present in 190 cases (51.9%) class Ⅱ in 123 cases (33.6%), and the class Ⅴ in 38 cases (10.4%).
3) The ratio of make to female according to Pap. class were 114 : 71 in the class Ⅰ.
71 : 51 in the class Ⅱ, 11 : 20 in the class Ⅲ, 3 : 1 in the class Ⅳ. and 28 : 8 in the class Ⅴ.
4) In the Pap. class Ⅳ. Ⅴ cases, percentage of positivity to examined number of sputum cytology was highest in the area from 91% to 100%, as 11 cases (26.2%), but the distribution covered a wide area from 11% to 80%.
5) Among the 403 cases examined by sputum cytology, class Ⅳ. Ⅴ were 42 cases, and their positivity in 4th successive sample was 35 cases (90.4%).
6) Of 34 cases dianosed by the histologic typing, the highest case was spuamous carcinome as 17 cases(50%), Adeno ca. was 7 cases (20.6%) 1 cases(2.9%).
7) In the ratio of male to female histologic typing, that of squamous carcinoma was 14 : 3 with male preponderance.
That of adenocarcinoma was 3 : 4 and that of oat cell carcinoma was 8 : 1.
8) In the percentage of positivity to examined number according to histologic type, the positivity of oat cell carcinoma was high, that of adenocarcinoma was low, but that of squamous carcinoma covered a wide area.
9) In positive cases in number according to type of cancer, initial positive number of squamous carcinoma covered from 1st to 9th, that of oat cell carcinoma finished at 2nd.
In summery the positivity of the sputum was lower over the 5th sputum samples but it is thought that sputum examination after 5th sputum samples were needed according to individuals.
Sputum cytology was easier than other method, and has important role in the diagnosis of the lung cancer.
