Purpose: To introduce a modified four-point transscleral intraocular lens fixation method and evaluate its safety and clinical outcomes.
Methods: We retrospectively reviewed the medical records of patients who underwent transscleral fixation from March 2015 to February 2020. Vitrectomy was performed, and a foldable intraocular lens was inserted through the corneal wound and fixed at four scleral points 2.0 mm posterior from the limbus. We analyzed the visual acuity, best corrected visual acuity, spherical equivalent, intraocular pressure, and complications at preop and 1 day, 1 month, 2 months, and 6 months postoperatively.
Results: In this study, 61 eyes of 59 patients were enrolled, and the mean follow up period was 12.68 ± 8.10 months. Best corrected visual acuity (logarithm of the minimal angle resolution) was improved and statistically significant at 1 month, 2 months, and 6 months postoperatively compared to preoperative values (all p < 0.001, paired t-test). Spherical equivalent changed to myopic and was significantly different at all periods compared to preoperative values (all p < 0.001, paired t-test). Prediction error was -0.77 ± 0.82 diopters at 6 months after the operation and tended to shift toward myopia. Macular edema occurred at one eye, but no retinal detachment, endophthalmitis, suture erosion, optic capture, intraocular lens tilt, or decentration was detected.
Conclusions: This modified four-point transscleral intraocular lens fixation surgical method is an easy procedure with excellent clinical outcomes and minimal complications.

국문 초록 (Abstract)

목적: 변형된 후방 인공수정체 4점 공막고정술의 수술 방법과 임상적 안정성 및 임상 결과에 대하여 알아보고자 한다.
대상과 방법: 2015년 3월부터 2020년 2월까지 변형된 후방 인공수정체 4점 공막고정술을 받은 환자를 대상으로 후향적으로 의무기록을 분석하였다. 유리체절제술을 먼저 시행하고, 각공막 윤부로부터 2.0 mm 떨어진 네 개의 공막 지점을 설정하여 각막절개창을 통해 삽입된 접힘형 인공수정체를 고정하는 방법을 시행하였고, 수술 전, 수술 후 1일, 1개월, 2개월, 6개월에 나안시력, 최대교정시력, 구면렌즈대응치, 안압 등의 임상 결과와 합병증을 알아보았다.
결과: 총 59명 61안으로 평균 추적검사 기간은 12.68 ± 8.10개월이며 최대교정시력(logarithm of the minimal angle resolution) 은 수술 전과 비교하였을 때, 1개월, 2개월, 6개월에서 시력의 호전을 보였다(1개월, 2개월, 6개월 p < 0.001, paired t-test). 구면렌즈대응치(diopters, D)는 수술 전과 비교하였을 때 근시값을 보였고, 모든 구간에서 유의하게 감소하였다(all p < 0.001, paired t-test). 예측 오차(D)는 수술 6개월 후 -0.77 ± 0.82 D로 근시화되는 경향을 보였다. 수술 후 낭포황반부종이 1안에서 발생하였고, 망막박리, 안내염 등의 합병증이 발생한 경우 없었고, 봉합사 노출, 인공수정체 광학부의 동공포획, 인공수정체의 기울어짐과 중심이탈이 발생하지 않았다.
결론: 본 방법은 후방 인공수정체 4점 공막고정술의 변형된 형태로, 수술 방법이 간편하고 임상적으로 안정적이며, 합병증 또한 적어유용한 수술 방법이다.
