자발적으로 입원한 알코올 의존자의 변화동기 하위유형 = Motivational Subtypes in a Voluntary Inpatient Sample of Alcohol Dependent Individuals

국문 초록 (Abstract)

본 연구에서는 치료응종(compliance)과 치료효과를 향상시킬 수 있는 중재법 개발을 촉진시키고 환자의 동기수준에 맞는 치료법을 제공할 수 있도록 하기 위해 알코올 의존자의 동기 하위유형을 확인하고자 하였다. 한국어판 변화단계 척도(The Korean version of Stages of Change Readiness and Treatment Eagerness Scale: SOCRATES-K)를 90명의 알코올 의존자에게 실시하여 군집분석 한 결과 4개 집단이 확인되었다. 집단 구성원들은 숙고전 집단 14%(n=13), 숙고 집단 42%(n=38), 낙심 집단 12%(n=11), 실행 집단 31%(n=28)로 분포되었다. 인구통계학적 특성, 음주 과거력이나 정신병리 및 성격에 있어서 집단간에 의미 있는 차이는 없었다. 실행집단은 자신들의 알코올 의존성에 대해 매우 잘 알고 있고 자신들의 음주행동을 현재 변화시키고 있으며, 금주에 대한 자기효능감이 높았다. 낙심 집단은 자신들의 알코올 의존성에 대해 잘 알고 있지만 단주를 실천하지는 못하고 또한 금주에 대한 자기효능감도 낮았다. 숙고 집단은 알코올 문제에 대한 어느 정도의 인식을 가지고 있으며 절주하거나 단기간의 단주도 시도해 보는 등의 노력을 하지만 아직 자신이 알코올 의존자임과 조절음주가 불가능하다는 사실을 온전하게 받아들이지 못하고 갈등하고 있다. 숙고전 집단은 문제에 대한 인식수준과 실천 노력수준이 매우 낮고, 알코올 의존자인지에 대해서도 받아들이지 못하고 있으며, 금주에 대한 자기 효능감도 매우 낮다. 현재의 연구결과들은 SOCRATES-K가 자발적으로 입원한 알코올 의존자들을 임상적으로 의미 있는 하위유형으로 분류하는 데 사용될 수 있음을 시사하며, 또한 환자들의 동기 하위유형에 맞추어서 치료법을 제공하는 데 있어서 하나의 기준자료가 될 수 있음을 시사한다.
