응급센터에 내원한 협심증과 급성 심근경색증 환자에서 초기증상의 임상분석 = Clinical Analysis of Initial Symptoms between Angina Pectoris and Acute Myocardial Infarction

Background: The initial symptoms in patients to suspected acute myocardial infarction or angina pectoris is important for early diagnosis and prompt treatment.
Method: We retrospectively studied in 72 patients diagnosed initially as angina pectoris(AP) or acute myocardial infarction(AMI). Clinical features such as age, sex, seasonal distribution, diurnal distribution, initial and associated symptoms, duration of chest pain, initial electrocardiographic finding, initial echocardiographic finding, final diagnosis, and mortality were reviewed.
Results: We found that chest pain (19 cases. 82.6%) was most common in AP, and (40 cases, 81.6%) in AMI. Duration of chest pain within 30 minutes were 16 cases (69.6%) in AP, but duration of chest pain after 30 minutes were 29 cases (59.2%) in AMI, and mean value of chest pain was 20.6±67.1 minutes in AP and 64.5±141 minutes in AMI. Relationship between initial diagnosis and final diagnosis were 44 cases (89.8%) in final AMI from initial AMI and 13 cases (56.5%) in initial AP from final AP.
Conclusion: With the initial symptoms such as chest pain, we can make early diagnosis of acute myocardial infarction or angina pectoris, performed further evaluation and prompt treatment.
