Introduction: Vasopressors are a powerful class of drugs that elevate mean arterial pressure (MAP). Vasopressors and inotropic agents have significant complications, including hypoperfusion(particularly affecting the extremities), dysrhythmias Usually, excessive vasoconstriction with inadequate perfusion can produce inadequate perfusion of the extremities, dusky skin changes at the tips of the fingers We conducted the present study to assess the risk factors of inadequate perfusion of extremities during vasopressors treatment in severe septic shock patients. Methods in medical ICU of Chonbuk National University Hospital in Korea, we retrospectively reviewed patients. The patients were stratified into ischemia or Non-ischemia groups in severe septic shock patients or not. Results: A total of 73 patients, 43 ischemia-group and 30 control (non-ischemia) group The baseline characteristics except BMI (p<0.028) was not different between groups. The use of dobutamine (inotropics) and the management of renal replacement therapy were significantly different between groups (p<0.05). Conclusions: The use of vasopressors and its combined use may not influence on the development of peripheral ischemia in severe septic shock patients.
