A case of 52-year-old woman who was found to have isolated renal amyloidosis is reported. The patient suffered from generalized edema for 1 month and was presented with nephrotic syndrome. Renal biopsy was performed after admission.
The light microscopic fidings showed massive infitration of the glomerular tuft with formation of large nodules and disappearance of most nuclei. Some glomeruli are completely obliterated by amyloid fibrils. Amyloid deposits also involve interstitium and vessels of all sizes. Congo-red stain in polarized light showed apple green birefringence of amyloid. On immunohistochemical stain, antibodies to amyloid A component, Lambda and Kappa light chain are not stained.
