LeFort Ⅱ Osteotomy is designed to bring the central midface forward with the dentoalveolar component. It should be used principally for this purpose. It may, however, be extended along the infraorbital margin where deformity is affecting the whole anterior maxilla and part of the malar complex.
In these cases, we used LeFort Ⅱ osteotomy for correction of retruded maxilla, and sagittal split rami osteotomy and genioplasty for correction of protruded mandible, we approached through bilateral canthal incision and two vestibular incisions. After osteotomy on the maxillary complex we used miniplate and screw system to fix the anteriorly advanced naso-maxillary complex and then grafted small iliac block bones and particulated cancellous bone and marrow along along the osteotomy line.
Two jaw surgery combined with LeFort Ⅱ osteotomy could be carried out effectively for correction of mandibular prognathism, especially by the case of combined midfacial retrusion. Coupled with bilateral sigittal split rami osteotomy and genioplasty and rhinoplasty, Le-Fort Ⅱ osteotomy was carried out for the patients of mandibular prognathism and maxillary retrusion in this case report.
