광사선 치료시 Setup 오차에 따르는 Multi-leaf collimator의 Scallop penumbra 변화 효과 = The Dosimetric Effects on Scallop penumbra from Multi-leaf Collimator by Daily Patient Setup Error in Radiation Therapy with Photon

국문 초록 (Abstract)

목적 : 반복되는 환자 치료에서 환자의 Setup 오차가 Multi-leaf collimator의 scallop penumbra 에 미치는 효과와 이의 임상적 의미를 살펴보고자 하였다.
대상 및 방법 : MLC의 leaf 방향에 대해 0°, 15°, 30°, 45°, 60°, 75° block 모양을 MLC와 통상적인 block으로 만들었다. 팬톰내에서 필름 측정법으로 통상적인 블록과 MLC가 만든 치료면의 penumbra 차이를 비교하였다. 방사선 치료는 30회 반복할 때의 Setup 오차가 고려된 선량분포를 중첩방식으로 구하였다. 위치 변화값은 연구보고 수치를 근거하여 구하였다.
결과 : 평균 4mm Setup error를 갖는 모델에서 1cm 폭분해능을 갖는 MLC가 만든 penumbra은 기존 차페 블록에서 생긴 penumbra 보다 차페 모양에 0~3cm 증가하는 효과를 볼 수 있었다.
결론 : 치료 조사면을 결정할 때 MLC를 사용하더라도 ICRU 50에 따라 환자의 움직임을 고려하여 치료조사면을 결정하였다면, 기존의 방식과 동일하게 치료면을 정할 수 있으며, 필요에 따라 3mm의 여유를 두면 MLC 치료 조사면 내에 치료 부위가 충분히 포함된다.

다국어 초록 (Multilingual Abstract)

Purpose : To evaluate the clinical implications of scallop penumbra width that comes from multileaf collimator(MLC) effect by the daily routine patient setup error.
Materials and Methods : The angle of 0°, 15°, 30°, 45°, 60° and 75° inclined-radiation block and the MLC. Film dosimetry in the phantom were performed to measure penumbral widths of differences between the dose distributions from the cerrobend block and those of respect the MLC. The patient setup error effect on scallop penumbra was simulated with repeated for the cerrobend block generated field.
Results : There are penumbral widths of to 3mm difference between the dose distributions from two kind of field shaping tools. the conventianal block and the MLC with 4mm setup error model and resonlution of 1cm leaf at the isocenter.
Conclusion : We need not additive margin MLC. if planning target volume is selected according to the recommendation of ICRU 50. For particlar cases, we can include the target volume with less than 3mm additive margin.
