정신장애인을 위한 직업재활 활성화 방안 = Strategies for the Activation of Vocational Rehabilitation for Mentally Disabled

국문 초록 (Abstract)

입원위주의 만성 정신질환자 치료와 관리가 재활과 지역사회정신보건 중심으로 전환되기 시작한 1990년대 중반 제정된 정신보건법은 정신장애인들의 퇴원과 지역사회 거주기간을 증가 시켰고, 많은 정신장애인들의 사회적 기능을 높이고 삶의 질을 높이는데 기여해 왔다. 1995년 이후 지역사회정신보건센터와 사회복귀시설이 전국적으로 늘어나면서 정신장애인들에게 제공되는 재활 프로그램의 하나인 직업재활은 정신장애인과 가족의 욕구에 의해서 점차로 재활 프로그램의 중심으로 자리잡게 되었다. 이런 가운데 2000년부터 정신장애인이 장애인 범주에 포함되고, 「장애인고용촉진및직업재활법」이 개정되어 바야흐로 정신장애인의 직업재활이 활성화될 수 있는 전기가 마련되었으나, 사회적 편견과 차별 속에서 정신장애인의 장애인 등록율은 여전히 50%를 밑돌고 있고, 미등록 정신장애인의 경우 장애인고용촉진 공단의 다양한 서비스를 제공받지 못하는 상황이 지속되고 있다. 또한 기초생활보장법에 의하여 소득이 있는 정신장애인의 의료급여가 제한을 받는 상황에서 정신장애인의 직업재활은 활성화되기가 어려운 것이 현실이다. 이러한 현실적 문제를 해결하기 위해서는 우선, 장애인등록자에 한정된 공단의 직업재활 서비스를 지원을 장애인을 증명할 수 있는 진단서와 소견서를 갈음할 수 있도록 지침을 변경하고, 정신보건시설에서의 의뢰체계를 확립하여 기관간 연계가 잘 이루어질 수 있어야 한다. 정신장애인의 재발을 막기 위해서는 직업을 통한 소득 발생 시, 기초생활보장법 수급권자에서 즉각적으로 탈락시키지 않고 일정기간 의료급여를 제공하는 특례를 시행하여 직업생활을 장려해야 할 것이다. 또한 입원시설, 지역사회 정신보건시설 등 정신장애인이 서비스를 받고 있는 시설을 장애인시설로 인정하여 이에 대한 고용촉진공단의 공단의 지원을 제공하여야 한다. 아울러 정신장애인을 위한 공동 사례관리와 정신장애인 직업재활 전문가 교육 프로그램의 도입 등도 직업재활 활성화를 위해 시급히 도입되어야 할 과제가 될 것이다.

다국어 초록 (Multilingual Abstract)

In the early 90th, psychiatric treatment for the chronic mentally ill patients were changed into the psychosocial rehabilitation and community mental health services, which has increased discharge rates and community tenures and improved quality of life of mentally disabled people. Numerous community mental health centers and rehabilitation facilities established since 1995 provided vocational rehabilitation services as a vital program among many psychosocial rehabilitation services. Registration of mentally disabled people made them beneficiaries of Welfare Law for the Disabled and receiving vocational program by Korea Employment Promotion for the DisabIed(KEPAD).
But social stigma and discrimination prohibited many mentally disabled people from registration that below 50% of total mentally disabled enrolled. Even the enrolled mentally disabled who has job and income getting out of MedicaI-Aid Program that many mentally disabled are worry about their relapse and rehospitalization..
To activate the vocational rehabilitation of psychiatric disabled, the author suggest several policies. First, strictly required registration of disabled people for the KEPAD services should be loosened that psychiatrists and other mental health professionals' reports can be substituted the registration card. Second, linkage between mental health services and vocational services of KEPAD must be developed and strengthened. Third, there should be Work Incentive Program for the hired mentally disabled who want MedicaI-Aid Program for continuous psychiatric care and medication. Forth, support and services from KEPAD should be available in the psychiatric institutes, community mental health centers and rehabilitation centers. Fifth, for the successful job maintenance, joint case management program should be provided by mental health service agency and KEPAD. Sixth, training program for the vocational specialist for the mentally disabled provided for the successful vocational rehabilitation of mentally disabled.
