명칭 실어증 환자의 어휘 판단에서 단어 빈도 및 길이 효과 = Word Frequency and Length Effects on the Lexical Decisions of Adults with Anomic Aphasia

Objectives: The goal of this study was to investigate the effects of word frequency and word length on the word comprehension of adults with anomic aphasia using a lexical decision task (LDT). Methods: The participants consisted of 25 adults with anomic aphasia (AA) and 25 normal adults (NA) with matching gender and educational levels. They performed a LDT that contained words of high- or low-frequencies and of two or four syllables. Results: The AA group showed significantly lower accuracy and longer reaction times (RTs) compared to the NA group in their performances on the LDT. The main effects of word frequency and word length were significant in both the performance accuracy and the RT, that is, the participants' performance accuracies were higher and their RTs were shorter when they encountered words that were of high-frequency and of two-syllable lengths. Particularly, both effects were greater in the AA group than in the NA group; however, only in relation to the RTs. Significant interaction between word frequency and word length was also observed. Conclusion: The effects of word frequency and word length on normal adults' lexical processing have been repeatedly reported. It was observed in this study that such effects were greater in the AA group than the NA group. It is suggested that the lexical processes of adults with anomic aphasia are fundamentally similar to those of normal adults. Nevertheless, adults with anomic aphasia are less effective at accessing lexical-semantic information.

국문 초록 (Abstract)

배경 및 목적: 본 연구는 명칭 실어증 환자들의 어휘 처리 과정이 정상 집단과 차이가 있는지 살펴보고, 음성 산출 전 어휘 접근 단계에 영향을 미치는 요인으로 알려진 단어 및 빈도 효과가 나타나는지 알아보고자 하였다. 방법: 연구대상은 명칭실어증 환자 25명과 학력, 성별, 연령을 일치시킨 정상 성인 25명이었다. 실험에 사용된 어휘판단과제는 비단어 30문항을 포함한 90개 문항으로 구성되었고, 빈도(고/저)와 길이(2음절/4음절)를 통제한 4 조건에 대하여 각 15개의 단어를 선정하였다. 결과: 연구 결과, 명칭 실어증 집단은 정상 집단에 비해 유의미하게 낮은 반응 정확도와 긴 반응 시간을 보여 어휘 처리의 어려움이 확인되었다. 특히 반응 시간 측정치에서 빈도와 길이 요인의 상호작용이 나타났으며, 두 요인 각각의 효과는 명칭 실어증 집단에 더 많은 영향을 미치는 것으로 관찰되었다. 논의 및 결론: 명칭실어증 환자들도 정상 성인과 유사한 어휘접근 경로를 거치며 단어 빈도나 길이와 같은 특성에 영향을 받지만, 언어 손상으로 인해 그 속도와 정확성 면에서 어려움을 겪는 것으로 보인다. 본 연구의 결과는 명칭 실어증 환자들이 보이는 이름대기의 어려움이 어휘-의미 정보와 관련된 접근 또는 인출 단계의 문제와 관련될 수 있음을 시사하였다.
