당뇨병 환자 교육이 정기적인 진료와 약물 복용, 생활습관의 관리 등을 이끌
어 내어 혈당을 효율적으로 관리하고 목표혈당을 유지하게 될 것이라는 가설
하에 당뇨병 환자의 목표 혈당조절 여부를 관찰하였다. 2019년 1월 14일부터
서비스를 개시한 보건복지부의 일차의료 만성질환관리 시범사업에 참여한 당뇨
병 환자 49,212명을 대상으로 국민건강보험공단의 자격DB, 만성질환 시범사업
DB 자료를 SAS Enterprise Guide 7.1을 이용하여 분석하였다. 당뇨병 관리교
육 경험, 사회 인구학적 특성(성별, 연령, 거주 지역, 소득수준, 의료보장 형태
등) 환자 관리 특성(이환기간, 가족력, 고혈압 동반여부 등) 시범사업 이행 특
성(이행단계, 자가 혈당 측정 횟수, 환자관리 횟수)요인이 혈당 조절 여부에 미
치는 영향을 분석하였으며 주요 연구결과는 다음과 같다.
단변량 모델에서 당뇨병 관리교육을 6∼10회 받은 군이 1∼5회 받은 군보다
혈당 조절군이 될 확률이 높았으나, 통제변수로 투입한 다변량 모델에서는 통
계적으로 유의하지 않았다. 당뇨병 환자의 교육이행 특성에 따른 당뇨병 관리
교육 경험을 비교분석한 결과를 살펴보면 교육경험이 연간 6∼10회인 집단의
연간 누적교육시간이 19분 미만에 해당하는 비율이 42.8%로 시범사업에서 권
장한 교육시간보다 부족한 것으로 나타나, 단지 교육 횟수만으로는 교육시간
과 정비례하지 않아 추후 교육의 질 관리에 대한 필요성도 나타났다.
‘당뇨병 이환기간’이 1년 미만인 집단과 비교하여 혈당 조절군이 될 확률은
1∼5년 된 집단이 0.827배, 6∼10년 된 집단이 0.642배, 10년 이상 된 집단이

0.521배로 더 유의하게 낮아지는 것을 확인하였다(p<0.001). 당뇨병 이환기간
이 길수록 혈당 조절이 잘 되지 않는 것으로 확인되므로 당뇨병 진단 초기에
당뇨병 자가 관리를 위한 환자교육을 실시해야 할 것으로 생각된다.
고혈압이 없는 집단 보다 고혈압을 동반하고 있는 집단이 혈당 조절군이 될
확률이 1.108배 더 높게 나타났다(p<0.001). 이는 고혈압 관리와 치료를 위한
행위가 혈당 조절에까지 영향을 끼친 것으로 유추해 본다.
케어플랜+교육+환자관리를 이행한 집단과 케어플랜+교육만 이행한 집단에
비해 케어플랜+교육+환자관리+점검평가를 모두 이행한 집단이 혈당 조절군이
될 확률이 유의하게 더 높은 것으로 나타났다(p<0.05). ‘자가 혈당 측정정도’에
서는 연간 9회 이상 측정한 집단과 비교하여 1∼4회 측정한 집단에서 혈당 조절
군이 될 확률이 1.107배 더 높고(p<0.001), ‘환자관리 누적횟수’에 따라서는 연간
3∼4회 이행한 집단과 비교하여 0회 이행한 집단에서 혈당 조절군이 될 확률
이 0.718배 낮은 것으로 나타났다(p<0.001).
성별, 거주 지역, 운동 유무, 영양/식생활 상태, 당뇨병 이환기간, 당뇨병 가
족력, 고혈압 동반여부, 합병증 검사(안저검사), 시범사업 이행단계, 자가 혈당
측정 횟수, 환자관리 누적횟수가 혈당 조절에 유의하게 영향을 미치는 요인으
로 확인되었다.
당뇨병 환자의 올바른 자가 관리를 위해서는 진단 초기부터 질병상태를 인
식하고 스스로 관리를 지속할 수 있도록 하는 것이 필요하다. 이를 위한 효과
적인 방법은 당뇨병 환자교육 단독 투입보다 의사의 케어플랜 하에 환자교육,
관리, 점검, 평가 등 의원에서의 적극적인 중재를 통한 체계적인 관리방안이
더욱 효과적임을 시사하고 있다.

Under a hypothesis that diabetes mellitus education would lead to efficient
glycemic control and helps maintain target blood glucose level through periodic
diagnosis, medication and habit management, this study looked into whether
diabetes patients reached the target glucose level. Using SAS Enterprise Guide 7.1,
this paper examined the beneficiary profile DB of National Health Insurance
Service, and DB of pilot project of chronic disease for 49,212 diabetes patients
who participated in the pilot project of primary care chronic disease management,
which commenced on January 14, 2019. We analyzed how glycemic control is
influenced by factors such as diabetes mellitus education, sociodemographic
characteristics (gender, age, area of residence, income level, and type of
healthcare, etc.), patient management characteristics (duration of diabetes, family
history, hypertension, etc.), and the characteristics of pilot project implementation
(implementation phase, the number of self-monitoring blood glucose, the number
of patient management). Major research results are as follows.
In a univariate regression model, a group of patients who received diabetes
mellitus education six to ten times was more likely to become glycemic control
group than the other group who received it one to five times. However, in a
multivariate model that used as a control variable, such finding was not observed
with statistical significance. The analysis of diabetes mellitus education by
characteristics of its implementation showed that the share of people who received
such education six to ten times a year and with annual cumulative education time
being less than 19 minutes was 42.8%, falling short of the number of hours of
education recommended by the pilot project. This proved that merely the number
of education sessions are not directly proportional to the number of hours of
education, suggesting the importance of managing the quality of education.
The probability of a group of diabetes patients with diabetes duration of one to
five years to become glycemic control group has 0.827 times that of a group of
diabetes patients with duration of diabetes being less than one year, the group
with diabetes duration of six to ten years was 0.642 times, and the group with
diabetes duration of ten years was 0.521 times, showing the declining probability
(p<0.001). This finding confirms that the longer the duration of diabetes, the less
effective glycemic control, suggesting that at the early stage of diabetes, education
for self-monitoring glucose level needs to be provided.
The group with high blood pressure was found to be 1.108 times more likely
to become a glycemic control group than the group without high blood pressure
(p<0.001). It can be inferred that patients’ efforts to manage and treat high blood
pressure affected even their blood glucose level.
The group who received a comprehensive package of care plan, education,
patient management and assessment was more likely to become glycemic control
group than the group who underwent care plan, education and patient
management, and the group who performed only care plan and education (p<0.05).
With regard to the level of self-monitoring blood glucose, the group who
measured glucose level one to four times a year was 1.107 times more likely to
become glycemic control group than the other group who measured nine or more
times a year, (p<0.001).
As for the cumulative number of patient management , the group who did not
receive it at all is 0.718 times less likely to become glycemic control group than
the group who received it three to four times a year (p<0.001).
Gender, place of residence, physical exercise, nutrition/and dietary conditions,
duration of diabetes, family history of diabetes, presence of hypertension,
inspection of complication (fundoscopy), phase of pilot project implementation,
the number of self-monitoring blood glucose, and the cumulative number of
patient management were related to glycemic control with statistical significance.
To ensure diabetes patients perform a correct self-monitoring of blood glucose,
it is necessary that they recognize the status of disease at the early phase and
continue to manage themselves. To do this, rather than diabetes patient education,
a more systematic management plan that supports active intervention such as a
combination of patient education, management, inspection and assessment under the
care plan of doctors is likely to be more effective.
