Cerebrovascular reserve is a general description of the range of cerebral perfusion variation from normal baseline. It indicates the level of stenosis of the blood vessels. For people with low cerebrovascular reserve, exercise or stress can radically increase the risk of stroke or ischemic attack. In this study, decrease in cerebrovascular reserve caused by internal carotid artery stenosis was predicted based on simulation by using a 3D cerebrovascular model. Then we observed the correlation between the cerebrovascular reserve and the anatomical variations of the circle of Willis. For the study, we made a cerebrovascular 3D grid and did blood flow simulation by a Navier-Stokes equation. As boundary conditions for simulation, input to the inlet is the pulsating pattern of arterial pressure, and, in each branch, lumped parameter model was combined to determine the outlet pressure. Therefore, we showed that several variations of circle of Willis increase risk at internal carotid artery stenosis.
