장애인은 구강건강관리에 있어 여러 면에서 매우 불리한 여건에 처해 있다.
구강건강상태가 양호하지 못함에도 불구하고 치료율은 상대적으로 낮은 경향이며, 특히 응급치료를 요하는 경우가 많다는 사실이 지적되고 있다.
치과의료 분야는 예방과 조기 진료가 그 무엇보다도 중요하다. 그러나 우리나라에서 예방과 조기진료 분야에서의 장애인 치과진료는 너무나 열악한 현실이다.
장애인은 비장애인에 비해 일반적인 치과 진료 군에 속하는 치주치료 수복치료 근관치료와 외과치료의 경우 1.4~3배에 달하는 치료시간의 차이를 보이며 이는 모두 통계적으로 유의미한 차이를 보인다.
장애인에게서 특히 중요한 예방진료는 우리나라의 사회보험적 성격의 의료보험 때문에 예방진료를 중요시해야 함에도 불구하고 현재는 질병 발생 후의 진료 위주로 정책을 택하고 있는 불합리한 상황에 놓여있다.
본 연구는 2014년 2월부터 12월, 2015년 2월부터 12월까지 서울시립장애인치과병원에 내원한 환자들의 병원자료 분석을 통해 장애인의 예방치과진료 실태를 알아보고 건강보험의 비급여 본인부담률 완화에 따른 예방치과진료의 변화를 찾아보려는 목적으로 연구하였다.
비급여 예방치과진료는 불소도포, 스켈링, 실런트를 대상으로 하였다.
비급여 예방치과진료를 받은 환자의 병원자료를 이용해 다음과 같은 결론을 도출하였다.
건강보험의 비급여 본인부담률 완화 후 예방치과진료의 이용이 증가 할 것으로 생각하였으나, 연구 결과 예방치과진료의 평균이용건수에서 본인부담률 완화 전후 차이가 없는 것으로 보였으며 예방치과진료의 평균금액 또한 환자가 완화라고 느낄 만큼의 감소폭이 아니었으며, 이로 인해 예방치과진료 이용에 큰 영향을 미치지는 못한 것으로 보인다.
이러한 연구를 통해 장애인의 예방치과진료의 실태를 파악할 수 있었으며 특히 정신적 장애인에게 예방치과진료가 얼마나 중요하고 개선되어야 할 부분이 많음을 알수 있었다.
추후 연구에서는 장애인의 예방치과진료에 대해 설문조사를 통해 포괄적인 연구가 이루어져야 할 것으로 생각된다.

The disabled are under difficult situations in oral health management in many facets. Although oral health status is not good, it is noted that medication rate tends to relatively be low and that in particular, emergency treatment is required for many cases. Preventable and early treatments are important in the dental medication field. However, in the fields of preventable and early treatments in Korea, dental treatment for the disabled is distinctively inadequate in reality. In cases of periodontal treatment, restorative treatment, root canal treatment and surgical treatment belonging to the general dental treatment group, the average treatment time for the disabled comparing that of the normal showed 1.4~3 times longer, demonstrating a statistically significant difference. Due to our medical insurance with the character of social insurance, preventable treatment which is important for the disabled and should be regarded important though, is left in an absurd situation that currently the policy of providing treatment after disease occurrence is employed.
The purpose of this study was to explore the status of preventable dental medication for the disabled through analyzing the hospital data of patients who visited Seoul Municipal Dental Hospital for the Disabled between February and December 2014 and between February and December 2015, and find out change of preventable dental medication according to alleviation of non‐benefit dental benefits coverage rates of health insurance.

By using hospital data of patients treated with non‐benefit preventable dental medications, this study yielded the following conclusions. Although the use of preventable dental medications was expected to be increased after alleviation of non‐benefit dental benefits coverage rates of health insurance, as a result of this study, no difference in the number of cases of average use was found before and after alleviation of dental benefits coverage rates, and the scope of the average payment for preventable dental medications was not enough for patients to feel alleviated, all of which eventually failed to influence the use of preventable dental medications.
Through the study above, this study explored the status of preventable dental medications for the disabled, and in particular, found out how important the preventable dental medications for the mentally disabled were and that there were a lot to be improved. It is believed that there should be comprehensive studies in the future through a survey research on preventable dental medications for the disabled.
