가정환경수정을 통한 안전 위험요인 감소가 노인의 기능장애수준에 미치는 영향: 지역사회 통합돌봄사업을 중심으로 = The Effect of Safety Risk Factor Reduction through Home Modification on the Level of Functional Disability in the Elderly : Focusing on Community Care Project

Objective : This study was to investigate the effect of reducing safety risk factors in the elderly through home modification on the level of functional disability.
Methods : Among the subjects of the study, 127 elderly people aged 65 years or older who lived in Seo-gu, Gwangju Metropolitan City and received community care services, who had the conditions to receive home modification services, were selected as subjects. The study period was from January 2020 to August 2021, and the evaluation tool used the Korean version of the fall risk residential environment assessment (HOME FAST) and the residential environment problem checklist to identify safety risk factors in the home, and the subject's function The World Health Organization Disability Assessment List (WHODAS 2.0) was used to determine the level of disability. According to the evaluation result, home modification intervention was applied according to the subject's desire, and the effectiveness was examined through re-evaluation.
Results : As a result of analyzing the effect on the level of functional disability of the subjects, it was confirmed that the fall risk environment factors had a relatively higher impact than the general characteristics. showed a statistically significant difference (p<.001). The level of functional disability improved in all areas (p<.001), and among them, Life activities, Mobility and Participation showed the greatest change in order.
Conclusion : Through the modification of the home environment to meet the subject's needs, falls and residential environment problems were statistically significantly reduced, and it was confirmed that the change in the level of functional disability was also statistically significantly improved. In the future, by continuing the home modification service in the community care project, not only the elderly but also all people with disabilities, such as the disabled and children living in the community, participate in activities on their own to live a self-directed life. Efforts are needed to nurture experts, collaborate, and expand service support.

국문 초록 (Abstract)

목적 : 본 연구는 가정환경수정을 통한 노인의 안전 위험요인 감소가 기능장애수준에 미치는 영향을 알아보고자 하였다.
연구방법 : 연구 대상은 광주광역시 서구에 거주하고 지역사회 통합돌봄(Community care) 서비스를 받는 65세 이상의 노인 중, 가정환경수정 서비스를 받을 수 있는 여건이 갖추어진 127명을 대상자로 선정하였다. 연구 기간은 2020년 1월부터 2021년 8월까지였으며, 평가도구는 가정 내 안전 위험요인을 파악하기 위해 한글판 낙상 위험주거환경 평가(HOME FAST)와 주거환경문제 체크리스트를 사용하였고, 대상자의 기능장애수준을 알아보기 위해 세계보건기구 장애평가목록(WHODAS 2.0)을 사용하였다. 대상자의 욕구에 따라 가정환경수정 중재를 적용하였으며, 재평가를 통해 효과성을 살펴보았다.
결과 : 대상자의 기능장애수준에 미치는 영향을 분석한 결과, 일반적 특성보다는 낙상 위험환경 요소가 상대적으로 높은 영향을 미치고 있는 것으로 확인되었으며, 가정환경수정 전과 후 차이 결과에서는 낙상위험환경과 주거환경 문제 부분에서 통계적으로 유의한 차이를 보였다(p<.001). 기능장애수준은 모든 영역에서 향상되었는데(p<.001), 그중에서도 돌아다니기(8.74), 참여하기(8.11), 일상생활 활동(4.10) 영역 순으로 큰 변화량을 보였다.
결론 : 대상자의 욕구에 맞는 가정환경수정을 통해 낙상 및 주거환경 문제가 통계적으로 유의하게 감소하는 변화를 나타냈으며, 기능장애수준의 변화에서도 통계적으로 유의하게 향상되어 긍정적인 변화를 보이는 것으로 확인되었다. 지역사회 통합돌봄(Community care) 사업으로 영양, 운동, 돌봄 서비스 등 개인의 상태 개선뿐 아니라 가정환경수정은 대상자의 기능장애수준 향상에 도움이 되었고, 추후 맞춤형 가정환경수정 서비스 정착에 도움이 될 것으로 사료된다
