어린 가토에서 사시 수술 후 스테로이드 안약의 효과 = Efficacy of Topical Steroid for Treatment after Strabismus Surgery in Young Rabbits

국문 초록 (Abstract)

연구목적:사시 수술 후 스테로이드 안약을 사용하는 경우가 있으나, 소아에서는 스테로이 드 안약의 효과와 합병증에 대한 임상적 연구가 미미하다. 이에 저자들은 어린 가토를 대상으로 사시 수술 후 스테로이드의 점안이 염증 조절 및 안압에 미치는 영향을 알아보고자 하였다.
대상 및 방법:생후 6∼7주된 가토 12마리에서 양안 상직근 절제술을 시행하였다. 우안 (실험군)에는 ofloxacin안약과 1% prednisolone안약을, 좌안(대조군)에는 ofloxacin안약만을 점안하였다(1일 4회, 2주). 수술 후 1일, 1주, 2주, 4주째 결막 부종, 결막 충혈, 분비물의 양을 관찰하여 점수화하였으며, Tono-Pen으로 안압을 측정하였다.
결과:실험군에서 대조군에 비하여 결막 부종 및 충혈 정도가 수술 후 조기에 소실되는 경 향이 있었고, 분비물의 양은 두 군 모두에서 조기에 소실되었다. 수술 후 각 시기에서 결막 부종, 결막 충혈, 분비물의 양은 두 군 사이에 통계학적으로 차이 없었다(p＞0.05). 실험군에서는 수술 후 1주째부터, 대조군에서는 2주째부터 수술 전에 비하여 안압이 통계학적으로 의미 있게 상승하였다(p＜0.05). 실험군 10안 중 1안에서 안압 상승으로 인한 각막부종 및 각막신생 혈관이 관찰되었다.
결론:어린 가토의 사시 수술 후 1% prednisolone 안약의 사용은 안압 상승의 위험성이 있다.

다국어 초록 (Multilingual Abstract)

Purpose:Topical corticosteroids are occasionally prescribed after strabismus surgery but data on children are limited. This study was performed to investigate the anti-inflammatory effect and the ocular hypertensive response to topical steroid after strabismus surgery in young rabbits.
Material and methods:Bilteral resection of superior rectus muscle were performed on 12 rabbits with 6 or 7-week old. Right eye (experimental group:group E) was instilled a topical 1% prednisolone and ofloxacin, while left eye (control group; group C) was instilled a topical ofloxacin with four times daily from immediately after surgery to 2 weeks after surgery. At postoperative 1 day, 1, 2, 4 weeks, conjunctival chemosis, injection, and dis-charge were scored and intraocular pressure (IOP) was measured with Tonopen.
Results:The conjunctival chemosis and injection were intended to decrease in the group E, eye discharge was intended to decrease in both groups during early postoperative period. There was no sratistically significant difference in conjunctival chemosis, injection and dis-charge between two group (p.0.05). There was a statistically significant difference between preoperative and postoperative IOP (p,0.05, at 1, 2, 4 weeks in group E:at 2,4 weeks in group C). There was an experimental case that had corneal edema and neovas-cularization because of high IOP.
Conclusion:After strabismus surgery in young rabbits, instillation of topical 1% pred-nisolone may have a risk of increased IOP.
