Dental caries is a bacterially based disease that progresses when acid is produced by bacterial action. Pathological
factors, including multiple species of acidogenic bacteria, salivary dysfuncion, and dietary carbohydrates are related
to caries progression. Protective factors, which include antibacterials, salivary calcium, phosphate and proteins, salivary
flow, and fluoride in saliva can balance, prevent or reverse dental caries.
This article will introduce caries management by risk assessment (CAMBRA). It will summarize the scientific evidence
behind the “caries imbalance” concept, which is the key to CAMBRA in clinical practice, Caries Risk Assessment
(CRA) and Caries management protocols.
CRA contains three domains : clinical findings, risk/biological factors and protective factors, and caries management
protocols are applied by the caries risk level.
Implementation of caries management by risk assessment into daily dental practice is a reality, with improvements
yet to come.
