20세기 한국 소아백신임상시험과 연구윤리규정 변화에 대한 고찰 = Pediatric Vaccine Clinical Trials and Changes in Research Ethics Regulation in 20th Century Korea

국문 초록 (Abstract)

우리나라 백신 임상시험과 관련하여, 1990년대 이전까지는 외국 수입 백신을 대상으로 부작용을 관찰하는 정도의 연구였다면, 1990년대부터는 국내 개발 백신의 보건당국 허가를 위한 본격적인 임상시험이 시작되던 시기였다. 연구윤리 관련 규정이나 제도는 1987년 12월 28일 보건사회부 고시로 제정·공포된 의약품 임상시험 관리기준이 존재하였으나, 당시 통용되던 국제기준에는 미치지 못하는 미흡한 실정이었다. 1990년대 한국에서 시행되었던 소아 대상 백신 임상시험에서 아동의 취약성과 동의 취득 문제가 국회를 중심으로 제기되기 시작하면서, 언론 및 사회 전반에 대상자 보호의 중요성에 대한 인식이 시작되었고, 이에 대한 보건당국의 대응으로 의약품임상시험관리기준 개정, 취약한 대상자에 대한 정의, 신약 임상시험 사전 승인제도 구축 등이 이루어지게 되었다. 이를 계기로 우리나라는 연구대상자와 취약한 대상자를 실질적으로 보호하는 방향으로 한 걸음 더 나아가 최근에는 임상연구심사위원회의 심의 외에도 연구가 제대로 진행되고 있는지 자체 점검을 시행하기도 한다. 1990년대 소아 백신연구를 통해, 연구 관련 윤리규정과 제도마련의 시작과 변화는 사회적으로 문제가 되는 임상연구에서 발생한 연구자의 실수, 제도적 미비점에 대한 반성에서 출발하며, 최종적으로는 법률 규정과 제도가 강화되는 상호영향관계에 놓임을 알 수 있다. 앞으로는 이러한 법적 제도를 뒷받침하여 대상자의 권리와 복지를 최우선으로 고려할 수 있도록 하는 사회적 문화 형성이 필요하겠다.

다국어 초록 (Multilingual Abstract)

Vaccine clinical trials in Korea prior to the 1990s were limited to observing the ad- verse reactions of foreign, imported vaccines. The 1990s marked the beginning of clinical trials for domestically developed vaccines. In 1987, the Korean Ministry of Health and Social Affairs established research regulations related to the management of pharmaceu- tical trials; however, these regulations were not up to prevailing international standards. As the National Assembly raised issues of child consent and vulnerability in vaccine trials, Korean media and society at large began to recognize the importance of protect- ing human subjects, which finally prompted health authorities to revise research ethics guidelines in the 1990s. Since then, Korea has taken substantial steps toward protecting vulnerable research subjects. The pediatric vaccine research of the 1990s demonstrates that the initiation and evolution of research ethics, regulations, and research environment change originate from reflections on researchers’ mistakes or deficiencies. Further, they mutually influence each other to strengthen legal regulations and institutions.
