미국 퇴역군인과 군복무관련 질환 = Military Service Connected Health Hazards in Veterans of the United States of America

국문 초록 (Abstract)

미국 보훈부는 172개의 병원과 439개의 외래진료소, 131개의 Nursing Home, 40개의 Domiciliary, 206개의 Vet Center를 운영하고 있다. 보훈부 의료기관에서는 퇴역군인에 대한 교육, 예방, 치료 및 재활 활동을 활발히 진행하고 있다. 퇴역군인의 장기 건강장해를 파악하기 위한 연구를 수행하는데 여러 개의 데이터 베이스를 가지고 있다. 이러한 자료는 퇴역군인 관리를 목적으로 만들어졌지만 이러한 자료를 이용하여 표본을 설정하거나 감시체계나 생정통계의 추구 조사로 사용하고 있다.
미국은 군복무관련 질환에 대하여 과거부터 기록하여 왔고 보상에 대한 기록이 있다. 군복무 중에 발생하거나 악화된 부상과 질병에 대하여 불명예 제대를 하지 않고 10%이상 장애가 발생하면, 평생동안 매달 일정액의 현금을 지불한다. 장애 보상이 되는 질환은 고혈압, 당뇨, 빈혈, 동맥경화증, 관절염, 뇌출혈, 뇌경색, 기관지확장증, 결석, 간경화, 심장염, 간질, 나병, 암, 신장염, 정신병, 활동성 결핵, 위궤양 등 거의 모든 질환이 포함되어 있다.소음성 난청도 생기면 평생 보상을 받는다. 군복무 중에 발생하거나 악화되었다는 증거가 없고, 퇴역 후 일정 기간내 발생하여도 추정 군복무관련 질환으로 장애 보상 혜택을 받는다. 만성질환, 열대질환, 전쟁포로 관련질환, 머스타드가스 관련질환, 방사선 관련질환, 한랭손상, 고엽제 관련질환 및 걸프전 증후군 등이다. 1995년 9월 퇴역군인이 26,067,163명으로 이 중 8.54%인 2,235,675명이 장해 연금 혜택을 받고 있다.
1998년 9월 공법을 제정하여 전쟁 관련 질병과 퇴역 후 건강 문제에 대한 National Center의 설립을 계획하고 있다. 또한 군대에서 사용하는 각종 화학물질에 대하여 발암성을 검토하고 폐기하거나 대체하고 있다.

다국어 초록 (Multilingual Abstract)

The Department of VETERANS aFFAIRS (VA) maintains some large automated databases that provide the opportunity for studying long-term health effects for military service. The Beneficiary Identification and Record Locator Subsystem (BIRLS) is an excellent source of vital status information on veterans.
The VA Patient Treatment File (PTF) is a computerized hospital discharge abstract system of inpatient records.
Monetary benefits, called disability compensation, are paid to veterans who are disabled by injury or disease (direct service connection) incurred or aggravated during active military service. A chronic, tropical, prisoner of war related disease, or diseases (presumptive service connection) associated with exposure to radiation and mustard gas will be considered to have been incurred in the service even though there is no evidence of such disease during the period of service.
The service of the veteran must have been terminated through separation or discharge under conditions that were other than dishonororable. Disability compensation varies with the degree of disability and the number of dependents, and is paid monthly.
Many studies have been conducted to determine the association of various cancers with military service in Vietnam. Some diseases have been compensated for Vietnam veterans.
Health problems reported following the Gulf War include a wide variety of symptoms similar to those found in acute combat reaction, post-traumatic stress disorder, and chronic fatigue.
On Sep.1995, there are 26,067,163 veterans in U.S.A. and disability compensation are paid to the 2,235,675 veterans (8.54%).
Health problems associated with war have continued and in some ways intensified. Therefore, The United States developed a plan for establishing a national center for the study of war-related illnesses and post-deployment healthe issues.
