The syndrome of inappropriate secretion of antidiuretic hormone (SIADH) is one of the most common causes of hyponatremia and is a disorder in which water excretion is partially impaired because of the inability to suppress the secretion of antidiuretic hormone(ADH). There are many causes which lead to SIADH including malignant tumors, drugs, infections and neuropsychiatric disorders. Here, we describe a 79-year-old man who complained of dizziness and showed hypotonic hyponatremia. However, the patient showed relatively high urine osmolarity and urine sodium concentration, and he was diagnosed withS IADH. Colonoscopy showed a 3.5 cm sized ulcerative mass at proximal transverse colon and biopsy showed adenocarcinoma. To our knowledge, this is the first case of SIADH caused by a colon cancer in Korea.
