Angiokeratoma is a rare vascular malformation of the upper dermis characterized by red-blue, verrucous hyperkeratotic papules with a diameter of 2 to 10 mm. Recently, dermoscopy has become a very useful method for the preoperative diagnosis of vascular lesions including angiokeratoma. Herein, we report a case of angiokeratoma with characteristic dermoscopic patterns. A 43-year-old female presented with slightly pruritic, solitary, 0.8 cm-sized, purplish plaque on the left knee since childhood. Systemic examination revealed no abnormal findings. Dermoscopic patterns showed a blue to whitish veil covered with scales and red and black lacunae suggesting angiokeratoma. Histopathological findings of hyperkeratosis, acanthosis, and several dilated dermal vessels congested with red blood cells confirmed the suspicion of angiokeratoma. From the dermoscopic patterns and histopathologic findings, we diagnosed the case as angiokeratoma and the lesion was excised completely.
