The double pylorus, a fistulous communication between gastric antrum and duodenal bulb is a very rare anomaly. It appears to be an acquired lesion in the majority of case and mainly in males who are alcoholics, diabetics, chronic renal failure, or chronic lung disease. Recently, the author experienced a case of double pylorus in a 60-year-old man who did not complain of gastrointestinal symptom but had the past history of severe gastric ulcer after the use of anti-inflammatory agents. An esophagogastroduodenoscopy for general health inspection confirmed the presence of double pylorus without active infammatory lesion. The author report its case with a review of the literature.
