재정 및 서비스 전달체계상 공사혼합 정도에 따른 국가별 유형화 및 비교 = The Typology and Comparison of Public-Private Mix in Long Term Care Service: A Comparative Study in Finance and Service Delivery Systems of Long Term Care Service in USA, Sweden and Germany

국문 초록 (Abstract)

사회복지서비스의 재원이나 서비스의 공급 방식은 전달체계를 통하여 구성되는데, 이는 곧 해당 제도의 효율성이나 효과성에 직접적인 영향을 미치게 된다. 따라서 특정 복지서비스를 전달체계의 관점에서 분석하는 것은 곧 해당 제도에 대한 이해를 돕고 운영상의 특징과 운영결과 등을 예측하는 데에 의의가 있다. 본 논문은 장기요양보호제도를 도입하여 운영하는 국가들을 재정 및 서비스 전달체계상 공사혼합 관점에서 분석하여 몇 개의 그룹으로 구분하여 유형화하는 분석틀을 제시하고, 각 그룹에 속하는 국가들의 장기요양보호제도가 재정 및 서비스 전달체계의 측면에서 어떠한 특징을 갖는지 분석 및 비교하였다.
본 논문에서는 세 가지의 연구 질문을 중심으로 내용을 구성하였다. 첫 번째는 장기요양보호제도를 도입하여 시행하고 있는 국가들을 어떻게 유형화할 수 있는가 하는 문제이다. 복지 국가를 몇 가지 유형으로 구분하여 비교하는 유형화 연구는 지금까지 다양한 방법들을 통하여 이루어져 왔다. 하지만 지금까지 수행하여 온 연구들은 국가별로 시행되고 있는 사회복지제도 전체를 기준으로 특정 요소를 따라 유형화 하거나, 국가의 성격 및 정책 결정 요인 등 거시적인 요소들을 기준으로 한 연구들이 대부분이다. 사회복지 프로그램이 각 국가별 상황이나 특성에 맞춰 수정되어 적용되는 것을 고려해 본다면 미시적인 요소들을 기준으로 유형화하는 것이 보다 더 현실에 맞는 결과를 나타낼 수 있다고 생각한다. 따라서 공통적으로 운영하고 있는 복지서비스를 기준으로 재정 및 서비스 공급 전달체계를 각각 측정하여 국가들을 분류하였다. 두 번째 연구 질문은 재정 및 서비스 전달체계의 공사혼합 형태를 바탕으로 유형화한 장기요양보호제도들이 국가별로 어떠한 특징을 나타내는가 하는 문제인데, 분류된 나라들을 몇 개의 그룹으로 묶어서 그 중 대표적인 나라들을 하나씩 골라 각각의 재정 전달체계 및 서비스 전달체계의 현황과 특징들을 분석하였다. 그 결과 각 그룹들은 서비스 전달체계와 재원 전달체계의 공사혼합 정도에 있어서 뚜렷한 차이를 나타냈다. 마지막 연구 질문은 한국의 장기요양보호제도가 다른 국가의 장기요양보호제도와 비교하여 어떠한 특성을 갖고 있는가이다. 결과적으로 재정 및 서비스 공급의 전달체계 상에 드러나는 차이를 통해 한국 장기요양보호제도의 특징을 다른 국가들과 비교하여 전달체계를 중심으로 파악할 수 있었고, 앞서 유형화한 국가들을 참고하여 전달체계 상의 공사혼합 정도를 조정할 수 있다면, 보다 효과적인 벤치마킹을 통하여 효율적인 제도 운영이 가능할 것으로 본다.

다국어 초록 (Multilingual Abstract)

This study explores the delivery system of Long term care service in various countries in terms of public-private mix. In order to study, how the Long term care service is being delivered in other countries, It classifies and also analyses 17 OECD countries according to the similar characteristics of the finance and service delivery systems of Long term care service.
Social welfare service is provided through a certain delivery system. It has a strong relation between the effectiveness and efficiency of the service and its delivery system for the success of the policy. Thus, how the finance and service is delivered and who delivers it, directly affects to the people who receive the service they need. Therefore, it is important analysing a Long term care service delivery system in terms of public-private mix. It helps a comprehensive understanding of the service and also be able to forecast outcomes of it.
The study showed a considerable difference in Long term care service in 17 different countries, in terms of which sector delivers the service and then how it is delivered. Amongst of all, the difference seemed to be even more distinctive in USA, Sweden, and Germany which are the representatives of three types of welfare states.
In order to manage Long term care service in Korea more effectively or efficiently, it may help understanding a various types of delivery systems of Long term care service in terms of public-private mix in other countries such as USA, Sweden, or Germany.
