A 52-year-old man presented with pruritic erythematous annular patches for 1 year, which were located on the extremities, mainly on both palms. Skin biopsy revealed a dense perivascular lymphohistiocytic infiltration with abundant eosinophils in the dermis. The results of laboratory test were within normal range, including eosinophils (0.7%). Several treatment options including systemic and topical steroids, and cyclosporine were administered. However, the lesion was only transiently improved and then aggravated. Therefore, hydroxychloroquine was initiated and continued for 4 months. The skin lesion and pruritus were improved significantly, without remarkable aggravation for 4 months.
